摘　　要：目的 分析IgG、补体C3联合子宫动脉血流在高危孕妇子痫前期诊断中的临床应用价值。方法 选取2021年7月—2022年7月于克拉玛依市中心医院门诊进行产检的250例具有子痫前期高危因素的孕妇为研究对象,最终发展为子痫前期的168例孕妇作为病例组,另外82例非子痫前期孕妇作为对照组。检测两组孕妇孕11~14周、17~20周、20~24周、30~34周免疫球蛋白G(IgG)、补体C3水平,并于同期行子宫动脉血流检测。结果 与对照组孕11~14周、17~20周、20~24周、30~34周相比,病例组孕11~14周、17~20周、20~24周、30~34周IgG、补体C3水平下降,子宫动脉血流搏动指(PI)、阻力指数(RI)值上升(P<0.05)。与IgG、补体C3、子宫动脉血流PI、RI值单项诊断相比,四项联合对子痫前期诊断价值较高曲线下面积为0.867(95%CI:0.815~0.920,P<0.05)。结论 IgG、补体C3、子宫动脉血流联合对子痫前期有较高的诊断价值,可提高子痫前期早期临床诊断准确率,有利于母婴结局的改善。Objective To analyze the clinical value of IgG,complement C3 combined with uterine artery blood flow in the diagnosis of preeclampsia in high-risk pregnant women.Methods A total of 250 pregnant women with high risk factors of preeclampsia who underwent obstetric examination in the outpatient department of Karamay central hospital from July 2021 to July 2022 were selected as the research objects, and 168 pregnant women who eventually developed preeclampsia were selected as the case group, and the other 82 non-children Preeclamptic pregnant women served as a control group.The levels of 11-14 weeks, 17-20 weeks, 20-24 weeks, 30-34 weeks immunoglobulin G(IgG) and complement C3 were tested in the two groups of pregnant women, and the uterine artery blood flow was tested in the same period.Results Compared with the control group pregnant 11-14 weeks, 17-20 weeks, 20-24 weeks, 30-34 weeks, the case group pregnant 11-14 weeks, 17-20 weeks, 20-24 weeks, 30-34 weeks IgG,complement C3 levels decreased, the uterine artery blood flow pulse index(PI),resistance index(RI) values increased(P<0.05).Compared with the single diagnosis of IgG,complement C3,uterine artery flow PI,and RI value, the four combination had a high diagnostic value for preeclampsia the area under the curve is 0.867(95% CI:0.815~0.920,P<0.05).Conclusion The combination of IgG,complement C3 and uterine artery blood flow has a high diagnostic value for preeclampsia, which can improve the accuracy of early clinical diagnosis of preeclampsia, and is beneficial to the improvement of maternal and infant outcomes.
