无框架立体定向植入导尿管头端弦尾在脑内肿瘤切除术中的应用

摘　　要：目的 探讨无框架立体定向下植入导尿管头端弦尾技术在基层医院脑内肿瘤手术中的临床疗效。方法 在无框架立体定向仪引导下,将带有弦尾的导尿管头端作为靶点定位肿瘤边界,切除大脑半球不同部位脑内肿瘤7例,均靠近或位于功能区。对本术式的手术疗效、优越性及注意事项进行分析。结果 全组病例术后6个月内间隔1个月以上的两次颅脑MRI检查对比证实均达到肿瘤影像学全切除,除1例不完全运动性失语外,余病例4周内临床症状均得到改善,肢体感觉运动等重要神经功能较术前未受明显影响,短期随访(<3个月)期间无复发。结论 无框架立体定向植入导尿管头端弦尾技术切除脑内肿瘤,具有影像学全切程度高、操作简便、设备价格低廉、所用耗材制作简单等优点,值得在基层医院神经外科推广。Objective To explore the clinical therapeutic effect of implanting the urinary catheter tip with a string tail guided by frameless stereotactic apparatus in intracerebral tumor resection in primary hospital.Methods Guided by frameless stereotactic apparatus,the catheter tip with a string tail was used to target the tumor boundary,and 7 tumors in different parts of the brain were removed,all close to or located in the functional area.The surgical efficacy,superiority and matters needing attention of this operation were analyzed.Results Whole group of cases have accepted two cranial brain MRI examinations with intervals of more than 1 month within 6 months after surgery,and achieved total tumor imaging resection after comparison confirmed.Except for one case of incomplete motor aphasia,clinical symptoms improved in the remaining cases within 4 weeks,important neurological functions such as limb sensorimotor were not significantly affected than preoperative ones,and there was no recurrence during short-term follow-up(<3 months).Conclusion This method of implanting the urinary catheter tip with a string tail guided by frameless stereotactic apparatus has the advantages of high full cutting degree of tumor imaging,simple operation,low equipment price,simple production of consumables used,which is worth promoting in the neurosurgery department of primary hospitals.
