胰岛素泵治疗老年糖尿病患者效果分析

摘　　要：目的探讨胰岛素泵治疗老年糖尿病患者的效果。方法选取本院收治的老年糖尿病患者104例,采用蓝白双色球法将其分为两组各52例,对照组采用多次皮下注射胰岛素治疗,观察组采用胰岛素泵治疗,比较两组的疗效、血糖水平、血糖波动情况、胰岛素抵抗指数、血糖达标时间、胰岛素用量、低血糖发生率以及不良反应发生率。结果观察组总有效率(96.15%)与对照组(92.31%)比较无明显差异(P>0.05);观察组1个月治疗期内的空腹血糖、餐后2 h血糖、睡前血糖、血糖波动均低于对照组(P<0.05);治疗前,组间24 h葡萄糖、糖化血红蛋白、胰岛β细胞功能指数以及胰岛素抵抗指数对比差异不具有统计学意义(P>0.05);治疗后,观察组24 h葡萄糖低于对照组,其余指标均高于对照组,差异具有统计学意义(P<0.05);观察组胰岛素用量低于对照组,血糖达标时间短于对照组(P<0.05);观察组低血糖发生率(3.85%)低于对照组(15.38%)(P<0.05);观察组不良反应发生率为5.77%(3/52),低于对照组的19.23%(10/52)(P<0.05)。结论胰岛素泵治疗老年糖尿病患者疗效确切,能够更加安全、快速、有效的控制血糖,维持血糖水平稳定。Objective To investigate the effect of insulin pump on elderly diabetic patients.Methods One hundred and four elderly diabetic patients in our hospital were selected and divided into two groups and 52 cases by blue and white double chromosphere.The control group was treated with multiple subcutaneous insulin injection,and the observation group was treated with insulin pump.The efficacy,blood sugar level,blood sugar fluctuation,islet resistance index,blood glucose reaching standard time,insulin dosage of the two groups were compared.The incidence of hypoglycemia and adverse reactions.Results There was no significant difference in the total effective rate between the observation group(96.15%)and the control group(92.31%)(P>0.05).The fasting blood glucose,postprandial blood glucose,bedtime blood glucose,blood glucose fluctuation in the observation group were lower than those in the control group(P<0.05).Before treatment,24 hours glucose,glycosylated hemoglobin and isletβThere was no statistically significant difference in cell function index and insulin resistance index(P>0.05).After treatment,the glucose in the observation group was lower than that in the control group at 24 hours,and the rest were higher than that in the control group,the difference was statistically significant(P<0.05).The dosage of insulin in the observation group was lower than that in the control group,and the time of reaching the standard of blood glucose was shorter than that in the control group(P<0.05).The incidence of hypoglycemia in the observation group(3.85%)was lower than that in the control group(15.38%)(P<0.05).The incidence of adverse reactions in the observation group was 5.77%(3/52),lower than 19.23%(10/52)in the control group(P<0.05).Conclusion Insulin pump is effective in the treatment of elderly diabetic patients.It can control blood glucose more safely,quickly and effectively,and maintain stable blood glucose level.
