乙酰半胱氨酸联合纤支镜灌注盐酸氨溴索治疗慢性阻塞性肺疾病急性加重期的效果及对氧化应激指标、MIF、BCL-2表达的影响

摘　　要：目的探讨乙酰半胱氨酸联合纤支镜灌注盐酸氨溴索治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床效果。方法将2019年3月至2021年2月收治的50例AECOPD患者以随机数字表法分为对照组和观察组,每组25例。对照组采用纤支镜灌注盐酸氨溴索治疗,观察组在对照组基础上加用乙酰半胱氨酸治疗。比较两组的治疗效果。结果观察组的治疗总有效率为96.00%,高于对照组的76.00%(P<0.05)。治疗后,观察组的丙二醛(MDA)、活性氧(ROS)水平低于对照组,谷胱甘肽过氧化物酶(GSH-Px)水平高于对照组(P<0.05)。治疗后,观察组的巨噬细胞移动抑制因子(MIF)表达低于对照组,B淋巴细胞瘤-2基因(BCL-2)表达高于对照组(P<0.05)。结论乙酰半胱氨酸联合纤支镜灌注盐酸氨溴索治疗AECOPD的效果确切,可明显减轻氧化应激反应,调节MIF、BCL-2表达,值得推广。Objective To investigate the clinical effect of acetylcysteine combined with fiberoptic bronchoscopy infusion of ambroxol hydrochloride in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 50 AECOPD patients admitted from March 2019 to February 2021 were divided into control group and observation group by random number table method,with 25 cases in each group.The control group was treated with fiberoptic bronchoscopy infusion of ambroxol hydrochloride,and the observation group was treated with acetylcysteine on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 96.00%,which was higher than 76.00%in the control group(P<0.05).After treatment,the levels of malondialdehyde(MDA)and reactive oxygen species(ROS)in the observation group were lower than those in the control group,and the level of glutathione peroxidase(GSH-Px)was higher than that in the control group(P<0.05).After treatment,macrophage migration inhibitory factor(MIF)expression in the observation group was lower than that in the control group,and B-cell lymphoma-2(BCL-2)expression was higher than that in the control group(P<0.05).Conclusion Acetylcysteine combined with fiberoptic bronchoscopy infusion of ambroxol hydrochloride is effective in the treatment of AECOPD,it can significantly reduce oxidative stress and regulate MIF and BCL-2 expression,which is worthy of promotion.
