PDCA循环法在输血相容性检测室间质评管理中的应用效果

摘　　要：目的探讨PDCA循环法在输血相容性检测室间质评管理中的作用。方法收集2018—2019年(PDCA实施前)我院输血相容性检测共11次室间质评结果以及2020—2021年(PDCA实施后)共12次室间质评结果。2020—2021年应用PDCA循环法对室间质评过程进行持续改进。比较PDCA实施前、后科室人员培训考核结果;分析PDCA实施后国家卫生健康委临床检验中心室间质评结果。结果PDCA实施后科室人员的仪器设备保养维护、试剂有效期确认、细胞浓度配置、样品转EP管前确认标记、凝集强度判读正确、抗筛阳性结果的分析以及查看、分析下发成绩得分均高于PDCA实施前(P<0.05)。PDCA实施后,国家卫生健康委临床检验中心室间质评结果与标准结果一致。PDCA实施后,除抗体筛查能力等级评价由PDCA实施前的C级提升至B级外,其余室间质评项目均达到了A级。结论PDCA循环法在输血相容性检测室间质评管理中起重要作用,其可规范室间质评操作流程,提高检测和分析水平。Objective To explore the role of PDCA cycle in the management of external quality assessment of transfusion compatibility testing.Methods A total of 11 external quality assessment results of transfusion compatibility testing in our hospital from 2018 to 2019(before PDCA implementation)and 12 external quality assessment results from 2020 to 2021(after PDCA implementation)were collected.From 2020 to 2021,PDCA cycle was applied to continuously improve the process of external quality assessment.The results of training and assessment of department personnel before and after PDCA implementation were compared;the results of external quality assessment of National Center for Clinical Laboratories after PDCA implementation were analyzed.Results After PDCA implementation,the scores of equipment maintenance,confirmation of reagent validity period,cell concentration configuration,confirmation of marking before sample transfer to EP tube,correct interpretation of agglutination intensity,analysis of positive results of anti-screening,and examination,analysis the results issued were higher than those before PDCA implementation(P<0.05).After PDCA implementation,the results of external quality assessment of National Center for Clinical Laboratories were consistent with the standard results.After PDCA implementation,except that the evaluation of antibody screening ability was upgraded from C grade before PDCA implementation to B grade,the rest of the external quality assessment projects reached A level.Conclusion PDCA cycle plays an important role in the management of external quality assessment of transfusion compatibility testing,which can standardize the operation process of external quality assessment,improve the level of detection and analysis.
