摘 要:目的探析在胎儿畸形中应用二维联合实时四维超声对检出率的影响。方法对2020年1月至2022年1月期间来我院接受产前检查840名孕妇进行回顾性分析,对其中50名分娩畸形胎儿的产妇进行不同的检查方法。实施二维超声检查的孕妇归入二维组,实施四维超声检查的孕妇归入四维组,实施二维联合实时四维超声检查的孕妇归入联合组。将随访结果作为诊断的金标准,比较3组的检出准确率、漏诊率、体表畸形准确率、体内畸形准确率。结果50名孕妇分娩胎儿50例,均为畸形案例,其中体表畸形26处(52.00%),体内畸形24处(48.00%)。检出准确率对比,二维组、四维组、联合组分别为66.00%、80.00%、88.00%;漏诊率比较,3组分别为34.00%、20.00%、12.00%;体表畸形准确率比较,3组分别为65.38%、84.62%、92.31%;体内畸形准确率比较,3组分别为66.67%、92.31%、83.33%;可见,与二维组相比,四维组与联合组的诊断效能更高,差异有统计学意义(P<0.05)。结论临床在诊断胎儿畸形中,二维联合实时四维超声所提供的检查图像更为全面、实时、立体,可以检查出胎儿畸形的具体情况,诊断效果明显,对于降低出生缺陷起到一定作用,值得临床推广与应用。Objective To explore the effect of two-dimensional combined real-time four-dimensional ultrasound on the detection rate of fetal malformations.Methods A retrospective analysis of 840 pregnant women who came to our hospital for antenatal checkups between January 2020 and January 2022 was conducted,and 50 of them who delivered malformed fetuses were examined by different methods.They were set as the twodimensional group after two-dimensional ultrasound examination,the four-dimensional ultrasound examination as the four-dimensional group,and the two-dimensional combined real-time four-dimensional ultrasound examination as the combined group.Follow-up results were taken as the gold standard for diagnosis.The detection accuracy,missed diagnosis rate,surface malformation accuracy and internal malformation accuracy of the three groups were compared.Results 50 fetuses were delivered by 50 pregnant women,all of which were malformation cases,including 26 surface malformations(52.00%)and 24 internal malformations(48.00%).The detection accuracy of 2D group,4D group and combined group were 66.00%,80.00%and 88.00%,respectively.The rate of missed diagnosis was 34.00%,20.00%and 12.00%,respectively.The accuracy of body surface deformity was 65.38%,84.62%and 92.31%,respectively.The accuracy of internal malformation was 66.67%,92.31%and 83.33%,respectively.Therefore,compared with the two-dimensional group,the diagnostic efficacy of the four-dimensional group and the combined group was higher,and the difference was statistically significant(P<0.05).Conclusion In the clinical diagnosis of fetal malformations,the examination images provided by two-dimensional combined real-time four-dimensional ultrasound are more comprehensive,real-time and three-dimensional,which can detect the specific conditions of fetal malformations.The diagnosis effect is obvious,and it plays a certain role in reducing birth defects,which is worthy of clinical promotion and application.
