腹腔镜下行保留生育功能的异位妊娠保守治疗临床效果探讨

摘　　要：目的 对腹腔镜下保留生育功能的异位妊娠保守治疗效果进行观察。方法 选取2018年1月至2019年1月期间74例异位妊娠患者,随机分为观察组与对照组,各37例。对照组给予开腹输卵管切开取胚术治疗,观察组给予腹腔镜下输卵管切开取胚术治疗,对两组手术相关指标、术后输卵管通畅率、术后宫内妊娠率等指标进行比较。结果 观察组术中出血量为(36.08±6.93)m L、手术时间为(33.24±4.85)min、术后排气时间为(8.87±1.34)h、住院时间为(3.21±1.48)d、血β-HCG水平为(165.37±33.82)m U/m L,均低于对照组的(65.82±9.57)m L(、49.15±5.94)min(、27.42±3.49)h(、5.72±1.96)d(、381.64±43.57)m U/m L,差异显著(t=7.273、4.292、9.628、6.915、12.558,P <0.05);术后输卵管通畅率对比,观察组(91.89%)高于对照组(62.16%)(χ^(2)=7.863,P <0.05);观察组术后宫内妊娠率(86.49%)高于对照组(64.86%)(χ^(2)=5.934,P <0.05)。结论 对异位妊娠患者采用腹腔镜下行保留生育能力的保守方法治疗,具有创伤小、术后恢复快的优势,可有效疏通输卵管,提高了宫内妊娠率,治疗效果较好,值得推广应用。Objective To observe the effect of conservative treatment of ectopic pregnancy with fertility preservation under laparoscopy.Methods From January 2018 to January 2019,74 patients with ectopic pregnancy were selected and randomly divided into observation group and control group,each with 37 cases.The control group was treated with open salpingotomy and embryos,and the observation group was treated with laparoscopic salpingotomy and embryos.The indexes related to surgery,postoperative fallopian tube patency rate,postoperative intrauterine pregnancy rate and other indexes were compared between the two groups.Results In the observation group,intraoperative blood loss was(36.08±6.93)mL,operation time was(33.24±4.85)min,postoperative exhaust time was(8.87±1.34)h,hospitalization time was(3.21±1.48)d,bloodβ-HCG level was(165.37±33.82)mU/mL,which were lower than those of the control group(65.82±9.57)mL,(49.15±5.94)min,(27.42±3.49)h,(5.72±1.96)d,(381.64±43.57)mU/mL,the difference was significant(t=7.273,4.292,9.628,6.915,12.558,P<0.05).The postoperative fallopian tube patency rate was compared,the observation group(91.89%)was higher than the control group(62.16%)(χ^(2)=7.863,P<0.05).The postoperative intrauterine pregnancy rate(86.49%)of the observation group was higher than that of the control group(64.86%)(χ^(2)=5.934,P<0.05).Conclusion Laparoscopy is used to treat patients with ectopic pregnancy with conservative methods of preserving fertility.It has the advantages of less trauma and quick postoperative recovery.It can effectively dredge the fallopian tube and increase the rate of intrauterine pregnancy.The treatment effect is good,and it is worthy of popularization and application.
