育阴止崩汤辨证治疗崩漏(肾虚血瘀证)临床疗效及对基础体温、凝血功能的影响

摘　　要：目的:观察育阴止崩汤辨证治疗崩漏(肾虚血瘀证)临床疗效及对基础体温、凝血功能的影响。方法:采用随机数字表法将2019年2月至2021年4月兴国县妇幼保健院收治的76例崩漏患者分为两组,每组38例。对照组予炔雌醇环丙孕酮片治疗,观察组予育阴止崩汤辨证治疗,均连续治疗3个疗程。评估两组止血疗效、中医症候疗效;检测两组治疗前后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)水平,检查治疗后基础体温(BBT)情况;记录用药期间药物不良反应。结果:观察组止血疗效、中医症候疗效总有效率及BBT双相率为92.11%、94.74%、73.68%,明显高于对照组的73.68%、76.32%、57.89%(P<0.05)。两组患者治疗后的PT、APTT值均减小,FIB水平均升高(P<0.05),且观察组PT、APTT值小于对照组,FIB水平高于对照组(P<0.05)。治疗期间,观察组不良反应总发生率为2.63%,明显低于对照组的15.79%,差异有统计学意义(P<0.05)。结论:育阴止崩汤辨证治疗崩漏(肾虚血瘀证)的止血效果显著,可有效改善患者凝血功能指标,缓解临床症状,且副作用更少。Objective:To observe the clinical effect of yuyin zhibeng decoction in the dialectical treatment of metrorrhagia(kidney-deficiency blood stasis)and its influences on basal body temperature(BBT)and coagulation function.Methods:According to random number table method,76 patients with metrorrhagia admitted to Xingguo County Maternal and Child Health Hospital were divided into two groups between February 2019 and April 2021,38 cases in each group.The control group was treated with ethinylestradiol and cyproterone acetate tablets,while observation group was given yuyin zhibeng decoction for dialectical treatment.Both groups were continuously treated for 3 courses.The hemostasis efficacy and TCM syndromes efficacy in both groups were evaluated.The prothrombin time(PT),activated partial thromboplastin time(APTT)and fibrinogen(FIB)before and after treatment in both groups were detected.BBT conditions after treatment were examined.And adverse drug reactions during medication were recorded.Results:The total response rates of hemostasis efficacy and TCM symptoms efficacy,and biphase rate of BBT in observation group were 92.11%and 94.74%,73.68%,higher than those in control group(73.68%,76.32%,57.89%)(P<0.05).After treatment,PT and APTT in both groups were decreased(P<0.05),which were lower in observation group than control group(P<0.05).After treatment,FIB in both groups were increased(P<0.05),which were higher in observation group than control group(P<0.05).During treatment,the total incidence of adverse reactions in observation group was lower than that in control group(2.63%vs 15.79%),the difference is statistically significant(P<0.05).Conclusion:The hemostasis effect of yuyin zhibeng decoction is significant in the dialectical treatment of metrorrhagia(kidney-deficiency blood stasis),which can effectively improve coagulation function indexes and relieve clinical symptoms,with fewer side effects.
