摘　　要：目的探讨精神专科医院心理门诊首诊患者心理健康水平与心率变异性(HRV)之间的关系。方法回顾性收集2020年1月至2021年6月该院心理门诊接受并完成HRV与症状自评量表(SCL-90)检查的1496例首诊患者资料,按照SCL-90的评分标准,将患者分为心理健康组和阳性症状组,判断两组HRV的差异,探讨HRV与心理健康水平的关系,分析HRV各指标数据对个体阳性症状的影响。结果1496例首诊患者中心理健康组471例,阳性症状组1025例,SCL-90总体阳性发生率为68.5%。与阳性症状组比较,心理健康组年龄[(40.14±15.42)岁vs.(33.92±13.27)岁]更大,高频功率[HF,168.11(67.68,335.99)Hz vs.149.78(66.39,273.12)Hz]更高,低频功率(LF)/HF[1.29(0.66,2.75)vs.1.74(0.92,3.24)]更低,差异有统计学意义(P<0.05)。HF与SCL-90总分、躯体化、焦虑、恐怖、精神病性因子分呈负相关,而LF/HF与SCL-90总分及各因子分呈正相关(P<0.05)。logistic线性回归分析显示,HF对SCL-90的总分及各因子分有预测作用(P<0.05)。结论HF可能是个体心理状态改变初期的一个灵敏指标。Objective To explore the relationship between the mental health level and heart rate variability(HRV)of the first-visit patients in the psychological clinic of a psychiatric hospital.Methods The data of 1496 first-visit patients who received and completed the examination of HRV and Symptom Checklist 90(SCL-90)in the psychological clinic of the hospital from January 2020 to June 2021 were retrospectively collected,and measured with the heart rate variability analysis system QHRV1 and SCL-90.According to the scoring criteria of the SCL-90,the patients were divided into mental health group and positive symptom group.The difference of HRV between the two groups was judged to explore the relationship between HRV and mental health,and the impact of HRV index data on individual positive symptoms was analyzed.Results Among the 1496 first-visit patients,there were 471 cases in the mental health group and 1025 were in the positive symptom group.The overall positive rate of SCL-90 was 68.5%.Compared with the positive symptom group,the age of the mental health group[(40.14±15.42)years old vs.(33.92±13.27)years old]was older,the high-frequency power[HF,168.11(67.68,335.99)Hz vs.149.78(66.39,273.12)Hz]was higher,and the low-frequency power(LF)/HF[1.29(0.66,2.75)vs.1.74(0.92,3.24)]was lower(P<0.05).The HF was negatively correlated with the SCL-90 total score,somatization,anxiety,terror,and psychotic scores(P<0.05),while LF/HF was significantly positively correlated with SCL-90 total score and individual factor score(P<0.05).Logistic regression analysis showe d that HF could predict the total score and all factor scores of SCL-90(P<0.05).Conclusion HF may be a sensitive indicator of the initial change of individual mental state.
