摘 要:目的 研究超剂量口服补钾对重度低钾患者的疗效及安全性。方法 回顾性分析2018年1月—2021年10月仙居县人民医院急诊医学科收治的206例重度低钾患者的临床资料,根据口服补钾剂量分为超剂量组93例,常规剂量组113例,比较两组的疗效及安全性。结果 补钾3、6 h后超剂量组血钾均高于常规剂量组,差异有统计学意义(P<0.05)。超剂量组总补钾时间及补钾总液体入量少于常规剂量组,差异有统计学意义(t=46.957、22.268,P<0.05)。补钾期间,两组不良反应发生率比较,差异无统计学意义(P>0.05)。Spearman相关分析显示,口服补钾剂量与3、6 h血钾水平呈正相关(r=0.870、0.850,P<0.01),与总补钾时间、补液量、心衰发生率呈负相关(r=-0.738、-0.691、-0.148,P<0.05)。结论 基层医院超剂量口服补钾对重度低钾血症患者安全、有效。Objective To study the efficacy and safety of over dose oral potassium supplementation in patients with se⁃vere hypopotassium.Methods Retrospective analysis of the clinical data of 206 patients with severe hypokalemia treated in the Emergency Medical Department of Xianju County People´s Hospital from January 2018 to October 2021.According to the dosage of oral potassium supplement,93 cases were divided into overdose group and 113 cases were conventional dose group.The efficacy and safety of the two groups were compared.Results After 3 h and 6 h of potassium supplementation,the blood potassium in overdose group was higher than that in conventional dose group,and the difference was statistically significant(P<0.05).The total of potassium supplementation liquid intake and the amount of potassium supplement liquid in the overdose group was less than that in the conventional dose group,and the difference was statistically significant(t=46.957,22.268,P<0.05).During potassium supplement,the adverse reac⁃tion rates between the two groups were compared,and the difference was not statistically significant(P>0.05).Spear⁃man correlation analysis showed that the oral potassium supplemental dose was positively correlated with the blood po⁃tassium level at 3 h and 6 h(r=0.870,0.850,P<0.01).It was negatively correlated with total potassium supplementation time,fluid supplementation amount and incidence of heart failure(r=-0.738,-0.691,-0.148,P<0.05).Conclu⁃sion Overdose oral potassium supplementation in primary hospitals is safe and effective for severe hypopotassium patients.
