局部针刺运动对急性踝关节扭伤患者的治疗效果及生活质量的影响

摘　　要：目的观察局部针刺运动对急性踝关节扭伤(AAS)患者的治疗效果及生活质量的影响。方法选取AAS患者82例按随机数字表法分为对照组与治疗组各41例。对照组行常规治疗;治疗组在对照组的基础上采用局部针刺运动疗法。两组连续治疗2周。比较两组踝关节疼痛、踝关节功能、气滞血瘀证症状评分、生活质量、临床疗效、患侧足背动脉血流动力学参数。结果治疗后,两组踝关节疼痛视觉模拟量表(VAS)评分明显下降,Kofoed评分显著上升,治疗组以上指标改善更明显(均P<0.01);治疗后,两组气滞血瘀证症状评分明显下降,治疗组下降更明显(均P<0.01);治疗后,两组中文版健康调查简表(SF-36)评分显著上升,治疗组上升更明显(均P<0.01);治疗组的总有效率为97.56%,明显高于对照组的80.49%(P<0.05);治疗后,两组患侧足背动脉的收缩期峰值血流速度(SF)、平均血流速度(TAM)显著上升,阻力指数(RI)明显下降,治疗组SF、TAM、RI的改善更显著(均P<0.01)。结论局部针刺运动疗法治疗AAS的疗效明显,可有效减轻疼痛,改善踝关节功能和中医证候,提高患者的生活质量。Objective:To investigate the efficacy of local acupuncture exercise on acute ankle sprain(AAS)patients and its effect on life quality.Methods:82 AAS patients admitted to our hospital were selected and divided into the control group(41 cases)and treatment group(41 cases)according to random number table method.The control group received conventional treatment,and local acupuncture exercise therapy was used in the treatment group on the basis of the control group.Both groups were treated for 2 weeks.Ankle pain,ankle function,symptom scores of syndrome of blood stasis due to qi deficiency,quality of life,clinical efficacy,and hemodynamic parameters of the dorsal artery of the affected foot were compared between the two groups.Results:After treatment,the visual analog scale(VAS)score was obviously decreased and Kofoed score of ankle pain in both groups was significantly increased,but the improvement of above indexes was more obvious in the treatment group(P<0.01).After treatment,symptom scores of syndrome of blood stasis due to qi deficiency was significantly decreased in the two groups,but the decrease was more significant in the treatment group(P<0.01).After treatment,the Chinese version of health survey summary form(SF-36)score was significantly increased in both groups,but the increase was more significant in the treatment group(P<0.01).The total effective rate of the treatment group was 97.56%,which was significantly higher than 80.49% of the control group(P<0.05).After treatment,the peak systolic velocity(SF)and mean flow velocity(TAM)of the affected dorsal foot artery were significantly increased and the resistance index(RI)was significantly decreased in the two groups,but the SF,TAM and RI were more significantly improved in the treatment group(P<0.01).Conclusion:The efficacy of local acupuncture exercise in treating AAS patients is obvious,which can effectively relieve pain,improve ankle joint function and traditional Chinese medicine syndromes,and improve the life quality of patients.
