CT血管造影在α-硫辛酸联合高压氧治疗糖尿病下肢血管病变中扫描狭窄血管的应用价值

摘　　要：目的分析α-硫辛酸联合高压氧治疗糖尿病下肢血管病变(DLEVD)效果评估中CT血管造影(CTA)扫描狭窄血管的应用价值。方法选定本院内分泌与代谢病科2020年1月至2022年1月接诊的100例DLEVD患者,以随机法将其分组(每组50例),对照组给予常规治疗,观察组在对照组基础上给予α-硫辛酸联合高压氧治疗,对比2组临床疗效、下肢动脉管腔截面积、CTA扫描血管狭窄程度、下肢动脉钙化程度。结果观察组临床总有效率(96%)高于对照组(72%),观察组治疗后左右侧髂总动脉、左右侧股动脉、左右侧足背动脉下肢动脉管腔截面积均低于对照组,观察组治疗后轻度狭窄率(46%)、中度狭窄率(52%)均高于对照组(34%、40%),观察组治疗后重度狭窄率(2%)低于对照组(26%),观察组治疗后0级率(10%)、1级率(50%)均高于对照组(0、42%),差异均具有统计学意义(P<0.05)。结论α-硫辛酸联合高压氧可有效缓解DLEVD患者肢体麻木等症状,改善血管狭窄程度、下肢动脉钙化程度,缩小下肢动脉管腔截面积,效果显著,通过CTA可以对α-硫辛酸联合高压氧治疗效果作出准确评价。Objective To analyze the application value of CT angiography(CTA)scanning stenotic vessels in the treatment of diabetic lower extremity vascular disease(DLEVD)withα-lipoic acid combined with hyperbaric oxygen.Methods One hundred DLEVD patients admitted to the Department of Endocrinology and Metabolic Diseases of our hospital from January 2020 to January 2022 were selected and randomly divided into the control group and the observation group(50 cases in each group).The observation group was givenα-lipoic acid combined with hyperbaric oxygen therapy on the basis of treatment for the control group and the clinical efficacy,lower extremity arterial lumen cross-sectional area,vascular stenosis degree and lower extremity arterial calcification degree were compared between the two groups.Results The total clinical effective rate(96%)of the observation group was higher than that of the control group(72%),and the cross-sectional areas of the lower extremity arteries of the left and right common iliac arteries,left and right femoral arteries,and left and right dorsal foot arteries in the observation group were lower than those in the control group after treatment,the mild stenosis rate(46%)and moderate stenosis rate(52%)of the observation group after treatment were higher than those of the control group(34%,40%),and the severe stenosis rate(2%)of the observation group after treatment was lower than that of the control group(26%),the 0-grade rate(10%)and 1-grade rate(50%)of the observation group after treatment were higher than those of the control group(0,42%).The differences were statistically significant(P<0.05).Conclusion Alpha-lipoic acid combined with hyperbaric oxygen can effectively relieve symptoms such as limb numbness in patients with DLEVD,improve the degree of vascular stenosis and lower extremity arterial calcification,and reduce the cross-sectional area of lower extremity arterial lumen.CTA can provide accurate evaluation of the effect of it.
