摘 要:目的探讨磁共振三维短时反转恢复快速自旋回波成像(3D-SPACE-STIR)序列在臂丛神经损伤诊断中的价值。方法回顾性分析40例临床手术确诊臂丛神经损伤者,术前均采用臂丛神经常规序列和3D-SPACE-STIR增强扫描序列,3D-SPACE-STIR原始图像进行多平面重建(MPR)重组及最大密度投影(MIP)重建,与临床手术结果进行对比。结果与手术结果对比,患者有45根神经损伤,其中节前22根,节后17根,混合型6根;3D-SPACE-STIR序列在臂丛神经节损伤诊断灵敏度为91.1%(41/45),特异度为71.4%(10/14),准确性为85.0%(51/60);磁共振成像(MRI)诊断与手术结果差异无统计学意义(P>0.05),Kappa≥0.4表示两者一致性较强。结论3D-SPACE-STIR序列在臂丛神经损伤患者中具有较高的准确性,且评估灵敏度和特异度均较高,为临床诊治臂丛神经病变提供了直观可靠的影像学依据。Objective To explore the value of MR 3D short-time inversion recovery for rapid spin-echo imaging(3D-SPACE-STIR)sequence in the diagnosis of brachial plexus injury.Methods Forty patients with clinical surgery confirmed brachial plexus injury were retrospectively analyzed,using the routine sequence and 3D-SPACE-STIR enhanced scan sequence,based on 3D-SPACE-STIR raw images,multiplanar reconstruction(MPR)reorganization and maximun intensity projection(MIP)reconstruction were completed,and compared with the clinical surgical results.Results Surgical results showed that there were 45 nerve injuries,including 22 preganglia,17 postganglia and 6 mixed.By 3D-SPACE-STIR sequence,the diagnostic sensitivity of brachial plexus ganglion injury was 91.1%(41/45),the specificity was 71.4%(10/14)and accuracy was 85.0%(51/60)(P>0.05),Kappa≥0.4 indicated strong agreement.Conclusion The 3D-SPACE-STIR sequence has high accuracy in patients with brachial plexus injury,and has high assessment sensitivity and specificity,which provides an intuitive and reliable imaging basis for the clinical diagnosis and treatment of brachial plexus neuropathy.
