低张水成像联合多模态磁共振成像应用于十二指肠原发性肿瘤及肿瘤样病变诊断中的价值分析

摘　　要：目的探讨低张水成像联合多模态磁共振成像(MRI)应用于十二指肠原发性肿瘤及肿瘤样病变诊断中的价值。方法回顾性分析我院在2015年1月至2021年12月期间经病理诊断为十二指肠病变的44例患者资料,所有患者均接受了低张水成像联合多模态MRI检查,根据病理诊断结果将患者分为良性组(十二指肠肿瘤样病变)和恶性组(十二指肠原发性肿瘤病变),分别有28例、16例。记录2组的病理结果,分析对比2组的多模态MRI功能评分、多模态MRI形态评分。结果2组的表观弥散系数(ADC)值、时间-信号强度曲线(TIC)曲线类型分布差异有统计学意义(P<0.05)。2组的肿块形态、管壁柔软、黏膜、组织浸润、转移病灶对比差异有统计学意义(P<0.05)。结论低张水成像联合多模态MRI可根据肿块形态、管壁柔软度、黏膜情况等鉴别十二指肠原发性肿瘤与肿瘤样病变,其中原发性肿瘤的ADC值较肿瘤样病变低,且TIC以Ⅲ型为主。Objective To explore the value of hypotonic water imaging combined with multimodal MRI in the diagnosis of primary duodenal tumors and tumor-like lesions.Methods A retrospective analysis of 44 patients with pathologically diagnosed duodenal lesions in our hospital from January 2015 to December 2021 was selected as the observation object.All patients received hypotonic water imaging combined with multimodal MRI examination.According to the results of pathological diagnosis,the patients were divided into benign group(duodenal tumor-like lesions,n=28)and malignant group(duodenal primary tumor lesions,n=16),respectively.The pathological results of the two groups were recorded,and the multimodal MRI functional scores and multimodal MRI morphological scores were analyzed and compared between the two groups.Results There were significant differences in ADC value and TIC curve type distribution between the two groups(P<0.05).There were significant differences in mass morphology,soft wall,mucosa,tissue infiltration,and metastatic lesions between the two groups(P<0.05).Conclusion Hypotonic water imaging combined with multimodal MRI can distinguish primary duodenal tumors from tumor-like lesions according to the shape of the mass,the softness of the tube wall,and the condition of the mucosa.The ADC value of primary tumors is lower than that of tumor-like lesions,and the TIC curve is mainly typeⅢ.
