摘　　要：目的 探讨右美托咪定联合全凭静脉麻醉对卵巢肿瘤患者手术期间心率、麻醉效果及及苏醒时间的影响。方法 选取莆田学院附属医院2020年1月—2022年1月收治的卵巢肿瘤手术患者60例,根据患者意愿,分为对照组与观察组,各30例。对照组接受全凭静脉麻醉,观察组接受右美托咪定联合全凭静脉麻醉,比较两组术中心率、麻醉效果及苏醒时间。结果 观察组T_1、T_2、T_3的心率均低于对照组,差异有统计学意义(t=5.121、5.248、4.939,P<0.05);观察组呼吸恢复时间(9.30±2.10)min、苏醒时间(10.25±2.15)min与拔管时间(11.62±2.84)min均短于对照组,差异有统计学意义(t=3.443、5.334、4.076,P<0.05);观察组术后1 d与3 d的简易智能精神状态量表(MMSE)评分[(24.83±1.70)分、(22.51±1.75)分]高于对照组,差异有统计学意义(t=5.021、6.805,P<0.05);观察组术后2、12、24 h的视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(t=3.473、5.511、9.380,P<0.05);观察组术后不良反应发生率(6.67%)低于对照组,差异有统计学意义(χ^(2)=5.455,P<0.05)。结论 右美托咪定联合全凭静脉麻醉可使卵巢肿瘤患者手术期间心率波动减小,提升麻醉效果,加快术后苏醒,减轻对患者认知功能的影响。Objective To investigate the effects of dexmedetomidine combined with total intravenous anesthesia on the heart rate,anesthetic effect and wake-up time of ovarian tumor patients during surgery.Methods Sixty patients admit⁃ted to the Affiliated Hospital of Putian College for ovarian tumor surgery from January 2020 to January 2022 were se lected.According to the patients'wishes,they were divided into control group and observation group,with 30 cases in each.The control group received total intravenous anesthesia,and the observation group received dexmedetomidine combined with total intravenous anesthesia.The operative center rate,anesthetic effect and awakening time between two groups were compared.Results The heart rates of T 1,T 2 and T 3 in the observation group were lower than those in the control group,the difference was statistically significant(t=5.121,5.248,4.939,P<0.05).The respiratory recovery time(9.30±2.10)min,awakening time(10.25±2.15)min and extubation time(11.62±2.84)min in the observation group were shorter than those in the control group,the difference was statistically significant(t=3.443,5.334,4.076,P<0.05).The Mini Mental State Examination(MMSE)scores at 1 d and 3 d postoperatively in the observation group[(24.83±1.70)points,(22.51±1.75)points]were higher than those in the control group,and the differences were statis⁃tically significant(t=5.021,6.805,P<0.05).The Visual Analog Scale(VAS)scores at 2 h,12 h and 24 h postopera⁃tively in the observation group were lower than those in the control group,the difference was statistically significant(t=3.473,5.511,9.380,P<0.05);the incidence of postoperative adverse effects in the observation group(6.67%)was lower than that in the control group,the difference was statistically significant(χ^(2)=5.455,P<0.05).Conclusion Dexme⁃detomidine combined with total intravenous anesthesia can reduce heart rate fluctuations during surgery in ovarian tu⁃mor patients,enhance the anesthetic effect,accelerate postoperative awakening,and reduce the impact
