利福平耐药肺结核患者接受含贝达喹啉方案治疗对心电图QT间期的影响

摘　　要：目的 分析利福平耐药肺结核患者接受贝达喹啉方案治疗对心电图QT间期的影响。方法 随机选取2020年2月—2022年2月福建医科大学附属泉州第一医院收治的接受含贝达喹啉方案治疗的80例利福平耐药肺结核患者,对其治疗前后不同时段的心电图进行监测记录,计算QT值;分析患者治疗前后不同时段的QT值。结果 80例患者治疗前的QT值平均为(404.87±19.25)ms,不同年龄、性别、耐药类型、BMI的QT值比较,差异无统计学意义(P>0.05)。全部80例患者中,16例患者在治疗期间发生QT值延长≥60 ms,7例患者同时发生QT值≥500 ms。患者第36、48周末的QT值分别为(419.08±31.36)、(419.58±27.72)ms,与治疗前QT值(404.87±19.25)ms比较,差异有统计学意义(P<0.05)。患者第60、72周末的QT值与治疗前比较,差异无统计学意义(P>0.05);观察期间,全部患者均没有发生显著的室性心律失常。结论 在选择含贝达喹啉方案治疗利福平耐药肺结核患者时容易导致QT延长,随之增大,大约在完成疗程后6个月QT值会恢复。Objective To analyze the effect of rifampicin-resistant tuberculosis patients treated with bedaquiline regi⁃men on the electrocardiographic QT interval.Methods Eighty patients with rifampicin-resistant pulmonary tuberculo⁃sis treated with bedaquiline containing regimen admitted to Quanzhou First Hospital of Fujian Medical University from February 2020 to February 2022 were randomly selected.Their electrocardiograms were monitored and recorded at different time periods before and after treatment,and QT values were calculated;the QT values of the patients at dif⁃ferent time periods before and after treatment were analyzed.Results The average QT value of 80 patients before treat⁃ment was(404.87±19.25)ms,there was no statistically significant difference in QT values of different age groups,sex,drug resistance type and BMI(P>0.05).Of all 80 patients,16 patients had QT value extension≥60 ms during treat⁃ment,and 7 patients had QT value≥500 ms at the same time.The QT values of the patients at the end of the 36th and 48th week were(419.08±31.36)ms and(419.58±27.72)ms respectively,which were significantly different from those before treatment(404.87±19.25)ms,the difference was statistically significant(P<0.05).The QT value of patients at the end of the 60th and 72nd weeks was compared with that before treatment,the difference was not statistically sig⁃nificant(P>0.05).During the observation period,no significant ventricular arrhythmia occurred in all patients.Con⁃clusion The selection of bedaquiline containing regimens for the treatment of rifampicin-resistant tuberculosis pa⁃tients tends to lead to QT prolongation,and the QT values recover approximately 6 months after the completion of the course.
