摘  要:目的探究临床药师在干预抗菌药物使用中的效果及其作用。方法选取2020年6月~12月的处方13000张(临床药师干预前)及2021年1月~6月处方13000张(临床药师干预后)进行分析,比较临床药师干预前、后的抗菌药物使用率、抗菌药物一联使用率、抗菌药物二联使用率、一类切口预防率、抗菌药物处方不合理情况、前五位抗菌药物DDDs值。结果临床药师干预后,抗菌药物使用率、抗菌药物二联以上使用率、一类切口预防均下降至41.22%、6.12%、7.50%,抗菌药物一联使用率升高至93.92%(P<0.05);临床药师干预后,抗菌药物处方不规范、用药不适宜、超常处方比例及处方不合理总数从2.62%、2.02%、1.06%、5.70%下降至0.97%、0.75%、0.39%、2.11%(P<0.05);临床药师干预后,前五位抗菌药物乳酸左氧氟沙星氯化钠注射液、头孢呋辛酯片、盐酸左氧氟沙星注射液、注射用头孢呋辛钠、注射用五水头孢唑林钠的DDDs值均降低。结论临床药师干预能够降低抗菌药物使用率,减少不合理抗菌药物处方,促进抗菌药物合理用药,提高用药安全性。Objective To explore the effect and role of clinical pharmacists in intervening antimicrobial use.Methods A total of 13,000 prescriptions in our hospital from June to December 2020(before clinical pharmacist intervention)and 13,000 prescriptions from January to June 2021(after clinical pharmacist intervention)were selected for analysis,and the antimicrobial efficacy before and after clinical pharmacist intervention was compared.We compare the use rate of antibacterial drugs before and after the intervention of clinical pharmacists,the use rate of the first combination of antibacterial drugs,the use rate of the second combination of antibacterial drugs,the prevention rate of the first class incision,the irrational situation of the prescription of antibacterial drugs,and the DDDs value of the top five antibacterial drugs.Results After the intervention of clinical pharmacists,the use rate of antibacterial drugs,the use rate of more than two antibacterial drugs,and the prevention of first class incision all decreased to 41.22%,6.12%,and 7.50%,and the use rate of one antibacterial drug increased to 93.92%(P<0.05).After the intervention of clinical pharmacists,the prescriptions of antibacterial drugs are not standardized,inappropriate,the proportion of abnormal prescriptions and the total number of prescriptions that do not meet the requirements from 2.62%,2.02%,1.06%and 5.70%decreased to 0.97%,0.75%,0.39%and 2.11%(P<0.05).After the intervention of clinical pharmacists,the DDDs of the top five antibacterial drugs,levofloxacin lactate and sodium chloride injection,cefuroxime axetil tablets,levofloxacin hydrochloride injection,cefuroxime sodium for injection and cefazolin sodium pentahydrate for injection,all decreased.Conclusion Clinical pharmacist intervention can reduce the usage rate of antibiotics,reduce the irrational prescription of antibiotics,promote the rational use of antibiotics,and improve the safety of medication.
