摘　　要：目的 研讨二甲双胍与胰岛素对妊娠期糖尿病(gestational diabetes mellitus, GDM)患者血糖水平及产妇、围生儿并发症的影响。方法 选取2020年10月—2022年5月惠安县医院收治的84例GDM患者作为研究对象,并依据随机数表法分组,对照组(n=42)接受胰岛素治疗,观察组(n=42)基于对照组用药条件施加二甲双胍治疗,比较两组治疗前后的血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 hPG)、胰岛素抵抗指数(HOMAIR)]、产妇及围生儿并发症发生情况。结果 治疗前,两组3项血糖指标值(FPG、2 hPG及HOMA-IR)相比,差异无统计学意义(P>0.05);治疗后,观察组该3项血糖指标值均低于对照组,差异有统计学意义(P<0.05)。观察组产妇及围生儿的并发症发生率(7.14%和9.52%)均低于对照组(23.81%和28.57%),差异有统计学意义(P<0.05)。结论 二甲双胍与胰岛素配伍用于GDM患者的治疗,能更有效降低血糖水平,减轻胰岛素抵抗,减少产妇及围生儿并发症发生。Objective To investigate the effects of metformin and insulin on blood glucose level and maternal and peri⁃natal complications in gestational diabetes mellitus(GDM).Methods A total of 84 patients with GDM who were admit⁃ted to Hui'an County Hospital from October 2020 to May 2022 were selected as subjects,and the group was set up ac⁃cording to random number table method.The control group(n=42)was treated with insulin,and the observation group(n=42)was treated with metformin based on the medication conditions of the control group.Blood glucose levels(fast⁃ing blood glucose(FPG)),2 h postprandial blood glucose(2 hPG),insulin resistance index(HOMA-IR),incidence of maternal and perinatal complications were compared between the two groups before and after treatment.Results There was no statistically significant difference in 3 blood glucose indexes(FPG,2 hPG and HOMA-IR)between the two groups before treatment(P>0.05).After treatment,the three blood glucose indexes in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of complica⁃tions of maternal and perinatal in observation group(7.14%and 9.52%)was lower than that in control group(23.81%and 28.57%),and the difference was statistically significant(P<0.05).Conclusion The combination of metformin and insulin in the treatment of GDM patients can more effectively reduce blood glucose level,reduce insulin resistance,and reduce the occurrence of maternal and perinatal complications.
