摘 要:目的探讨RDW、PDW、呼出气一氧化氮(FENO)与肺功能联合检测在儿童哮喘诊断中的应用效果。方法选取2021年1月—2022年5月江苏省苏北人民医院收治的42例支气管哮喘患儿为观察组,同期42名接受体格检查的健康儿童为对照组。采集两组静脉血样测定RDW、PDW,一氧化氮分析仪测定两组呼出气一氧化氮(FENO),Master Screen肺功能仪测定两组肺功能指标第1秒用力呼气容积(FEV1)与用力肺活量(FVC),比较各单一指标及联合诊断儿童哮喘的效能。结果观察组RDW、PDW与FENO均高于对照组,FEV1与FVC均低于对照组,差异有统计学意义(t=8.332、10.352、14.229、7.135、14.447,P<0.05)。各项指标联合诊断的灵敏度(95.24%)、特异度(97.62%)、符合率(96.43%),均高于各指标单一诊断,差异有统计学意义(P<0.05)。结论RDW、PDW、呼出气一氧化氮与肺功能联合检测可以为儿童支气管哮喘临床诊断提供有价值的依据和参考,值得临床应用推广。Objective To explore the application effect of combined detection of RDW,PDW,fractional exhaled nitric oxide(FENO)and lung function in the diagnosis of asthma in children.Methods 42 children with bronchial asthma ad⁃mitted to North Jiangsu People's Hospital from January 2021 to May 2022 were selected as the observation group,and 42 healthy children who received physical examination during the same period were selected as the control group.RDW and PDW were determined by venous blood samples of the two groups,and FENO in exhaled gas was deter⁃mined by nitric oxide analyzer.The first second forced expiratory volume(FEV1)and forced vital capacity(FVC)of the two groups were measured by the Master Screen pulmonary function apparatus,Effectiveness of single and combined diagnosis of childhood asthma.Results The RDW,PDW and FENO in the observation group were higher than those in the control group,while FEV1 and FVC were lower than those in the control group,the difference was statistically sig⁃nificant(t=8.332,10.352,14.229,7.135,14.447,P<0.05).The sensitivity,specificity and coincidence rate of the com⁃bined diagnosis were 95.24%,97.62%and 96.43%,which were higher than those in the single diagnosis of each in⁃dex,the difference was statistically significant(P<0.05).Conclusion The combined detection of RDW,PDW,frac⁃tional exhaled nitric oxide and lung function can provide valuable basis and reference for the clinical diagnosis of chil⁃dren with bronchial asthma,and is worthy of clinical application and promotion.
