结核性脊柱炎与培养阴性化脓性脊柱炎的比较

摘　　要：目的比较结核性脊柱炎(TS)与培养阴性化脓性脊柱炎(CNPS)的临床及影像学特点。方法对123例TS及180例CNPS病人的临床资料进行回顾性分析,比较两组病人一般资料、实验室检查及影像学结果的差异。结果两组病人体温≥38℃、结核史、腰大肌脓肿、结核菌感染T细胞斑点试验阳性、结核分支杆菌γ-干扰素释放试验阳性病人占比及红细胞沉降率、降钙素原、C反应蛋白水平差异均有显著性(χ^(2)=6.929~49.578,P<0.05;t=2.818~18.142,P<0.05)。结论TS与CNPS的临床特征、实验室及影像学结果存在差异,据此可对病人进行及时的诊治。Objective To compare the clinical and imaging features of tuberculous spondylitis(TS)and culture-negative pyogenic spondylitis(CNPS).Methods A retrospective analysis was performed for the clinical data of 123 patients with TS and 180 patients with CNPS,and the two groups of patients were compared in terms of general data,laboratory examination,and imaging findings.Results There were significant differences between the two groups in the proportion of patients with body temperature≥38℃,a history of tuberculosis,psoas major abscess,positive results of T cell spot test for Mycobacterium tuberculosis infection or positive results of interferon-γrelease test for Mycobacterium tuberculosis,erythrocyte sedimentation rate,procalcitonin,and C-reactive protein(χ^(2)=6.929-49.578,P<0.05;t=2.818-18.142,P<0.05).Conclusion There are certain differences in clinical features,laboratory markers,and imaging findings between TS and CNPS,and timely diagnosis and treatment can be achieved based on such differences.
