高尿酸血症患者中医脉象分析及其与动脉粥样硬化的相关性研究

摘　　要：目的探讨高尿酸血症患者中医脉象分析及其与动脉粥样硬化的相关性。方法选取1575例高尿酸血症患者为研究组,另选取445例健康体检者为对照组。比较2组中医脉象参数[主波幅值(h1)、重搏前波幅值(h3)、降中峡幅值(h4)、重搏波幅值(h5)、一个心动周期(t)、动脉内压力高水平状态持续时间(W)、张力系数(h3/h1)、阻力系数(h4/h1)、弹性系数(h5/h1)、主波波幅上1/3宽度与一个脉动周期比值(W/t)]、内皮功能指标[一氧化氮(NO)、内皮素-1(ET-1)]及动脉脉搏波速度(PWV),采用Pearson相关性分析高尿酸血症患者中医脉象参数与其动脉粥样硬化的相关性。结果研究组h3、h4、h3/h1、h4/h1、t、W、W/t高于对照组(P<0.05),h1、h5、h5/h1低于对照组(P<0.05);研究组NO水平低于对照组(P<0.05),ET-1水平、PWV高于对照组(P<0.05);经Pearson相关性分析结果显示,h3、h4、h3/h1、h4/h1与NO呈负相关(P<0.05),与ET-1、PWV呈正相关(P<0.05);h1、h5、h5/h1与NO呈正相关(P<0.05),与ET-1、PWV呈负相关(P<0.05)。结论高尿酸血症患者的中医脉象参数改变,内皮功能降低和PWV加快,并且中医脉象参数与动脉粥样硬化有相关性。Objective To explore the correlation between traditional Chinese medicine(TCM)pulse analysis and arteriosclerosis in patients with hyperuricemia.Methods Totally1575 patients with hyperuricemia were as the study group,and 445 healthy people were as the control group.TCM pulse parameters(main wave amplitude[h1],pre pulse wave amplitude[h3],descending isthmus amplitude[h4],pulse amplitude value[h5],one cardiac cycle[t],duration of high level of intra-arterial pressure[W],tension coefficient[h3/h1],resistance coefficient[h4/h1],elasticity coefficient[h5/h1],the ratio of 1/3 width of the main wave amplitude to a pulse cycle[W/t]),endothelial function indicators(nitric oxide[NO],endothelin-1[ET-1])and arterial pulse wave velocity[PWV] were included as comparisons.Pearson correlation was performed to analyze the correlation between TCM pulse parameters and arteriosclerosis in patients with hyperuricemia.Results h3、h4、h3/h1、h4/h1、t、W、W/t in the study group were higher than those in the control group(P<0.05),while h1、h5、h5/h1 in the study group were lower than those in the control group(P<0.05).NO level in the study group was lower than that in the control group(P<0.05),while level of ET-1 and PWV were higher than those in the control group(P<0.05).Pearson correlation analysis showed that h3、h4、h3/h1、h4/h1 were negatively correlated with NO(P<0.05),and positively correlated with ET-1 and PWV(P<0.05).h1、h5、h5/h1 were positively correlated with NO(P<0.05),and negatively correlated with ET-1 and PWV(P<0.05).Conclusion The change of TCM pulse parameters in patients with hyperuricemia leads to the decrease of endothelial function or the acceleration of PWV,and TCM pulse parameters are positively correlated with arteriosclerosis.
