消炎散外敷治疗膝骨关节炎急性发作临床研究

摘　　要：目的观察消炎散外敷治疗膝骨关节炎(KOA)急性发作的临床疗效。方法将110例KOA急性发作患者按照随机数字表法分为2组,治疗组55例采用消炎散外敷,对照组55例采用氟比洛芬凝胶贴膏外敷,共治疗2周。比较2组治疗前后疼痛视觉模拟评分(VAS)、西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分、关节腔积液评分、C反应蛋白(CRP)、红细胞沉降率(ESR),并统计复发率和临床疗效。结果2组治疗后疼痛VAS均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后WOMAC骨关节炎指数各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后膝关节腔积液评分均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后CRP水平均较本组治疗前降低(P<0.05),2组治疗后比较差异无统计学意义(P>0.05)。2组治疗前后ESR水平比较差异无统计学意义(P>0.05)。治疗后3个月治疗组复发率低于对照组(P<0.05)。2组临床疗效比较差异无统计学意义(P>0.05)。结论消炎散能够有效缓解膝关节疼痛,改善膝关节功能,降低疾病复发率,为治疗KOA急性发作的有效外敷药。Objective To observe the clinical effect of Xiaoyan Powder on acute attack of knee osteoarthritis(KOA).Methods A total of 110 patients with acute knee arthrosis were randomized 1∶1 to receive flurbiprofen cataplasms(the control group)or Xiaoyan Powder(the treatment group)for 2 weeks.The score of visual analogue scale(VAS),Western Ontario and McMaster Universities Arthritis Index(WOMAC),and joint effusion,C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),recurrence rate,as well as clinical efficacy were included as comparisons.Results After treatment,the score of VAS,WOMAC,and joint effusion was decreased,which were decreased in the treatment group than in the control group(all P<0.05).CRP in groups was decreased(P<0.05),there was no significant difference between groups after treatment(P>0.05).The ESR levels in the two groups were comparable before and after treatment(P>0.05).The 3-month recurrence rate in the treatment group was lower than that in the control group(P<0.05),There was no significant difference in clinical efficacy between groups(P>0.05).Conclusion Xiaoyan Powder can effectively relieve knee joint pain,improve knee joint function,reduce recurrence rate,which is an effective external dressing for acute recurrence of KOA.
