穴位贴敷联合孟鲁司特钠片、布地奈德气雾剂对儿童支气管哮喘急性期患者肺功能及血清炎症因子水平的影响

摘　　要：目的观察穴位贴敷联合孟鲁司特钠片、布地奈德气雾剂对儿童哮喘急性期患者肺功能及血清炎症因子水平的影响。方法将108例儿童哮喘急性期患者按照随机数字表法分为2组,对照组54例予孟鲁司特钠片联合布地奈德气雾剂治疗,治疗组54例在对照组治疗基础上应用穴位贴敷治疗。2组均治疗4周,比较2组患儿治疗前后肺功能指标[用力肺活量(FVC)、第1 s用力呼气容积(FEV_(1))、峰值呼气流速(PEF)]、血清白三烯D4(LTD4)、神经生长因子(NGF)水平、动脉血气分析指标及炎症相关指标[白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)]水平,并统计疗效。结果治疗组总有效率83.33%(45/54),对照组总有效率68.51%(37/54),治疗组临床疗效优于对照组(P<0.05)。2组治疗后FVC、FEV_(1)、PEF均较治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。2组治疗后LTD4、NGF水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后PO_(2)、OI均较本组治疗前升高(P<0.05),PCO_(2)均降低(P<0.05),且治疗组治疗后PO_(2)、OI均高于对照组(P<0.05),PCO_(2)低于对照组(P<0.05)。2组治疗后血清IL-6、TNF-α、hs-CRP水平较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论穴位贴敷联合孟鲁司特钠片、布地奈德气雾剂治疗儿童哮喘急性期疗效确切,可改善肺功能,降低炎症因子水平,改善肺通气功能。Objective To investigate the effect of treating winter disease in summer with acupoint application therapy combined with montelukast sodium tablets,budesonide aerosol inhalation on pulmonary function and inflammatory factors in asthma children.Methods The asthma children(n=108)were randomly assigned into two groups.On the basis Montelukast sodium tablets+budesonide aerosol inhalation(the control group,n=54),and the treatment group(54 cases)received treating winter disease in summer with acupoint application therapy,the two groups were treated for 4 weeks.The pulmonary function index(forced vital capacity[FVC],forced expiratory volume in the first second[FEV_(1)],peak expiratory flow rate[PEF]),serum leukocyte C4(LTD4),nerve growth factor(NGF),as well as arterial blood gas analysis indexes and related inflammation-related indexes(interleukin-6[IL-6],tumor necrosis factor-α[TNF-α],high sensitivity C-reactive protein[hs-CRP]) were included as comparisons.Results After treatment,the clinical efficacy of the treatment group was better than that of the control group(total effective rate:83.33%[45/54]vs 68.51%[37/54],P<0.05).FVC,FEV_(1) and PEF in groups were higher(P<0.05),and LTD4 and NGF were decreased(P<0.05),the difference in the above indexes was statistically significant between groups(all P<0.05).PO_(2) and OI in groups were increased(P<0.05),and PCO_(2) was decreased(P<0.05),there were statistically significant difference in the above indexes between groups(all P<0.05).Serum IL-6,TNF-αand hs-CRP in groups were decreased(P<0.05),and which were lower in the treatment group than in the control group(P<0.05).Conclusion For asthma children,treating winter disease in summer with acupoint application therapy combined with Western medicine has a definite effect,which can improve lung function,reduce the level of inflammatory factors and improve lung ventilation function.
