两种皮瓣修复单指背侧肌腱外露创面的疗效对比

摘　　要：目的对比指动脉背侧支皮瓣和掌骨背皮瓣修复单指背侧肌腱外露创面的疗效。方法自2019年3月至2020年12月于我科治疗的单指背侧肌腱外露创面患者共32例,其中采用指动脉背侧支皮瓣修复14例(A组),掌骨背皮瓣修复18例(B组)。记录两组患者的性别年龄伤指指别、受伤侧别、受伤机制、创面面积、受伤至手术时间、手术时间、随访时间、皮瓣存活情况、皮瓣感觉恢复情况、患肢功能评定以及患者满意度评分。结果术后平均随访时间为A组(15.4±6.1)个月,B组(15.1±4.4)个月。两组的创面面积、受伤至手术时间、随访时间和皮瓣感觉恢复情况差异无统计学意义。所有皮瓣完全存活,功能评定A组优5例、良8例;B组优9例、良8例。A.B两组手术时间分别为(63.2±7.5)和(53.1±7.5)min,B组短于A组,差异有统计学意义(P<0.05)。A、B两组的患者满意度评分分别为7.1±1.0和8.2±0.9,B组高于A组,差异有统计学意义(P<0.05)。结论与指动脉背侧支皮.瓣相比,掌骨背皮瓣更适合修复单指背侧肌腱外露创面。Objective To compare theclinical efficacy of dorsal branch of digital artery flap and dorsal metacarpal flap in repairing the dorsal wound of single finger with tendon exposure.Methods From March 2019 to December 2020,a total of 32 patients with single digit dorsal tendon exposure wounds were treated in our department.Among them,14 patients were repaired with dorsal branch of digital artery flap(group A),and 18 patients were repaired with dorsal metacarpal flap(group B).The gender,age,injured finger,injured side,injury mechanism,wound area,time from injury to operation,operation time,follow-up time,survival of flap,sensory recovery of flap,functional evaluation of affected limb and patient satisfaction score were recorded.Results The average follow-up time was(15.4±6.1)months in group A and(15.1±4.4)months in group B.There was no significant difference in wound area,time from injury to operation,follow-up time and skin flap sensory recovery between the two groups.All the flaps survived completely.The result was excellent in 5 cases,good in8 cases for group A,and excellent in 9 cases,good in 8 cases for group B.The operation time of group A and B was(63.2±7.5)and(53.1±7.5)minutes,respectively.It of group B was shorter than that of group A,and the difference was statistically significant(P<0.05).The patient satisfaction scores of group A and B were 7.1±1.0 and 8.2±0.9,respectively.It of group B was higher than that of group A,and the difference was statistically significant(P<0.05).Conclusion Compared with the dorsal branch of digital artery flap,the dorsal metacarpal bone flap is more suitable for repairing dorsal wound of single finger with tendon exposure.
