摘  要:目的观察运动再学习联合经颅直流电刺激对脑梗死患者下肢运动功能的影响。方法将符合纳入标准的120例脑梗死患者采用随机数字表法分为三组,每组40例,A组给予运动再学习治疗,B组给予经颅直流电刺激治疗,C组给予运动再学习联合经颅直流电刺激治疗,5个疗程后进行下肢运动功能评定、日常生活活动能力评定及三维步态分析。结果三组患者下肢运动功能Fugl-Meyer评分、Barthel指数评分、三维步态分析时空参数和下肢关节运动参数治疗前后差值采用方差分析比较,差异均有统计学意义(P<0.05);组间两两比较,C组下肢运动功能Fugl-Meyer评分、Barthel指数评分、三维步态分析时空参数和下肢关节运动参数的改善程度均高于A组和B组,差异均有统计学意义(P<0.05)。结论运动再学习结合经颅直流电刺激可以提高脑梗死患者下肢运动功能和生活自理能力,纠正患者异常步态。Objective To observe the effect of motor relearning programme(MRP)combined with transcranial direct current stimulation on lower limb motor function of patients with cerebral infarction.Method Divide 120 patients with cerebral infarction according with the inclusion criteria into three groups(n=40)by random number table.Patients in group A receive MRP.Patients in group B receive transcranial direct current stimulation.Patients in group C receive MRP combined with transcranial direct current stimulation.Perform Fugl Meyer Assessment(FMA)on lower limb motor function,activity of daily living assessment and three-dimensional gait analysis after five courses.Result Difference of score on FMA of lower limb motor function,Modified Barthel Index(MBI),spatiotemporal parameters and motor parameters of lower limb joint in three-dimensional gait analysis before and after treatment between three groups analyzed by variance analysis is significant(P<0.05).Comparison between any of the two groups shows that improvement level on score on FMA of lower limb motor function,score on MBI,spatiotemporal parameters and motor parameters of lower limb joint in three-dimensional gait analysis in group C are higher than improvement level in group A and B(P<0.05).Conclusion MRP combined with transcranial direct current stimulation can improve lower limb motor function and activity of daily living of patients with cerebral infarction and revise their abnormal gait.
