七味白术散治疗小儿单纯性肥胖脾虚湿困证36例疗效观察

摘　　要：目的观察七味白术散治疗小儿单纯性肥胖脾虚湿困证的临床效果。方法选取2018年6月至2021年4月就诊于深圳市龙华区中心医院横坑社区健康服务中心的36例单纯性肥胖脾虚湿困证患儿,均予七味白术散配方颗粒口服,连续治疗3个月后,比较治疗前后中医证候积分、体质量指数(BMI)、糖代谢水平[空腹血糖(FBG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)]、血脂水平[甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)]和25-羟维生素D3[25(OH)D3]水平。结果治疗后中医证候积分、BMI、FBG、FINS、HbA1c、HOMA-IR、TG、LDL水平均明显降低,HDL、25(OH)D3水平明显升高,差异均有统计学意义(P<0.05或P<0.01)。结论七味白术散治疗小儿单纯性肥胖脾虚湿困证疗效显著,可有效调节机体糖脂代谢和类固醇激素代谢水平。Objective To observe the clinical efficacy of Qiwei Baizhu San(七味白术散) in the treatment of simple obesity in children with dampness of spleen deficiency symptom,and explore the possible mechanism.Methods A total of 36 children with simple obesity with dampness of spleen deficiency symptom who were admitted to Hengkeng Community Health Service Center,Longhua District Central Hospital of Shenzhen City from June 2018 to April 2021 were selected and given Qiwei Baizhu San formula granules orally.We kept statistics about traditional Chinese medicine (TCM) syndrome score,body mass index (BMI),glucose metabolism level [(including fasting blood glucose (FBG),fasting insulin (FINS),glycated hemoglobin (HbA1c),insulin resistance index (HOMA-IR)],blood lipid level [including triglyceride (TG),low density lipoprotein (LDL),high density lipoprotein (HDL)] and 25 (OH)D3 levels after 3 months of treatment.Results Compared with before treatment,TCM syndrome scores,BMI,FBG,FINS, HbA1c,HOMA-IR,TG and LDL levels were decreased significantly after treatment,while HDL and 25 (OH) D3 levels were increased significantly,with statistical significance(P<0.05 or P<0.01).Conclusion Qiwei Baizhu San can regulate the body’s glucose, lipid metabolism and steroid hormone metabolism effectively in the treatment of simple obesity in children with dampness of spleen deficiency syndrome, and the curative effect is significant.
