摘　　要：目的 探究老年患者住院费用的主要影响因素,为科学控制老年患者住院费用和减轻老年群体疾病经济负担提供参考依据。方法 从医院住院病案首页系统提取2018年1月1日-2020年12月31日出院的65岁及以上老年住院患者的基本信息与住院费用信息,运用描述性分析、单因素分析和多元线性回归方法对住院费用进行分析。结果 老年患者次均费用为12836.7元,其中占比最高的3项费用分别为药费(31.3%)、诊断费(25.7%)与综合医疗服务费(23.3%)。影响住院费用的主要因素有性别、医疗费用支付方式、住院天数、入院途径、是否转科、是否手术、出院情况和疾病类型。结论 住院天数、医疗费用支付方式、死亡结局和疾病类型是影响老年患者住院费用的重要因素,政府部门与医院应针对影响因素进行合理管控。提高医院综合管理水平与医疗质量,提升医保基金监管能力,大力发展医疗机构与社区临终关怀事业,加强老年群体呼吸系统与循环系统疾病防控是控制老年患者住院费用的关键。Objectives This study aims to explore the main influencing factors of hospitalization expenses in elderly patients, so as to provide a reference for scientific control of hospitalization costs for elderly patients and the reduction of the economic burden of disease in elderly groups.Methods This study extracted the basic information and hospital expense information of elderly inpatients aged 65 and above admitted from January 1, 2018 to December 31, 2020 from the hospital medical records front pages, and used descriptive analysis, univariate analysis, and multiple linear regression methods to analyze hospitalization costs.Results The average cost per time for elderly patients was 1,2836.7 yuan, of which the top three costs were medicine(31.3%), diagnosis(25.7%),and comprehensive medical service(23.3%). The main factors affecting hospitalization cost were gender, medical insurance payment modes, length of stay, admission route, whether to transfer to another department, whether to undergo surgery, discharge status, and type of disease.Conclusions Length of stay, medical insurance payment modes, death outcomes, and disease types are important factors that affect the hospitalization costs of elderly patients, and government departments and hospitals should manage and control the influencing factors reasonably.Improving the comprehensive management level and medical quality of hospitals, improving the supervision ability of medical insurance funds, vigorously developing medical institutions and community hospice care, and strengthening the prevention and control of respiratory and circulatory diseases in the elderly are the keys to controlling the hospitalization cost of elderly patients.
