摘 要:目的研究使用双能X射线吸收法(DXA)测定髋部骨密度时,股骨头和股骨颈交界区外、上部感兴趣区(ROI)缺失对全髋骨密度值及T值的影响,为扫描后处理提供数据支持。方法在2018年5月28日-2021年8月17日的符合检测技术标准并存在髋部ROI缺失的受检者中,根据标画前的T值,选取正常组、骨量减少组和骨质疏松组各50例。通过对缺失ROI进行人工标画,对比标化前后骨密度值、T值的变化,分析造成问题的原因及问题造成的影响,并提出改进方案。同时对2021年5月1日-5月31日接受DXA检测的所有门诊女性患者568例进行了髋部ROI缺失发生率的调查。结果髋部ROI缺失,会造成91.3%受检者的全髋骨密度增加,平均上涨5.0×10^(-3)g/cm^(2);6%受检者骨密度无变化;2.7%受检者骨密度降低,平均降低1.0×10^(-3)g/cm^(2)。30%受检者共45人全髋T值增加,其中44人T值增加0.1,1人T值增加0.2,其余70%受检者T值无变化。结论使用DXA进行髋部骨密度扫描时,ROI缺失会导致全髋骨密度增加,甚至造成T值的改变。为了给临床提供更可靠的诊断依据,应对缺失的骨缘进行人工标画。Objectives This study aims to study the effect of bone region of interest(ROI)deficiency of the femur on bone mineral density(BMD)and the diagnostic reference T-value of the total hip during BMD scanning by dual X-ray absorptiometry(DXA),so as to provide numerical evidence for the post-processing.Methods Among subjects who met the technical criteria for detection and had femur with ROI deficiency between May 28,2018 and August 17,2021,a normal group,osteopenia group,and osteoporosis group with 50 cases each were selected according to the T value By manually marking the missing ROI,comparing the changes of bone mineral density and T value before and after the standardization,this study analyzed the cause of the problem and the impact of the problem,and put forward an improvement plan.Results ROI deficiency of the total hip resulted in an increase in total hip BMD in 91.3%of the subjects,with an average increase of 5.0×10^(-3)g/cm^(2).6.0%had no change in BMD.2.7%of the subjects had a decrease in BMD,with an average decrease of 1.0×10^(-3)g/cm^(2).30.0%of the subjects had a total hip T value increase in 45 subjects,of which 44 subjects had an increase of 0.1 in their T value,1 subject had an increase of 0.2 in their T value,and the remaining 70.0%subjects had no change in their T value.Conclusions The ROI deficiency occurs frequently in femur scanning by DXA,which can lead to the increase of the BMD value of total hip,even changing the T-value of its.In order to provide reliable diagnostic data for clinical practice,the loss region of ROI should be mapped manually in the post-processing step.
