摘  要:目的 分析新生儿消化道出血的危险因素及与维生素K水平的相关性,为新生儿预防消化道出血提供参考。方法 选取2020年8月1日-2021年12月31日来医院住院治疗的新生儿2343例,通过医院信息系统收集所有患儿病例资料,根据临床表现和辅助检查统计新生儿消化道出血例数,根据新生儿是否发生消化道出血分为消化道出血组(n=138)和正常组(n=2205);统计患儿胎龄、性别、出生体重、出生日龄、分娩方式、窒息程度、合并颅内出血、维生素K水平;采用logistics回归分析新生儿发生消化道出血的独立危险因素,对新生儿消化道出血与维生素K水平和Apgar评分进行Pearson相关性分析;结果 2343例新生儿发生消化道出血138例(5.89%),其中男性61例,女性77例,其中早产儿39例,足月儿81例,过期儿18例;两组在合并颅内出血、出生体重、出生日龄、胎龄、Apgar评分和维生素K水平方面进行比较,差异有统计学意义(P<0.05);多因素logistic回归分析结果显示,合并颅内出血、重度窒息和维生素K缺乏是新生儿发生消化道出血的的独立危险因素(P<0.05);经Pearson相关性分析,新生儿发生消化道出血与维生素K水平和Apgar评分呈负相关(r=-0.448,-0.507, P<0.05)。结论 新生儿发生消化道出血的风险较高,对于维生素K缺乏、重度窒息及颅内出血的患儿,应尽可能提早给予干预,可降低新生儿发生消化道出血的风险。Objectives This study aims to analyze the risk factors of neonatal gastrointestinal bleeding and its correlation with vitamin K level, so as to provide a reference for the prevention of gastrointestinal bleeding in neonates.Methods A total of 2343 neonates hospitalized from August 2020 to December 2021 were selected.All cases were collected through the hospital information system.According to clinical manifestations and auxiliary examinations, the number of neonatal gastrointestinal bleeding cases was counted, and the neonates were divided into a gastrointestinal bleeding group(n=138) and a normal group(n=2205) according to whether neonates had gastrointestinal bleeding.The gestational age, gender, birth weight, age at birth, mode of delivery, degree of asphyxia, intracranial hemorrhage, and vitamin K level were analyzed.Logistic regression was used to analyze the independent risk factors of neonatal gastrointestinal bleeding, and Pearson correlation analysis was performed to analyze neonatal gastrointestinal bleeding, vitamin K level, and Apgar score.Results Among 2343 neonates, 138(5.89%) had gastrointestinal hemorrhage, including 61 males and 77 females, including 39 premature infants, 81full-term infants, and 18 expired infants. The two groups were compared in terms of intracranial hemorrhage, birth weight, age at birth, gestational age, Apgar score, and vitamin K level, and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that intracranial hemorrhage, severe asphyxia, and vitamin K deficiency were independent risk factors for neonatal gastrointestinal hemorrhage(P<0.05).According to Pearson correlation analysis, neonatal gastrointestinal bleeding was negatively correlated with vitamin K level and Apgar score(r=-0.448,-0.507,P<0.05).Conclusions Neonates have a high risk of gastrointestinal bleeding.
