摘 要:目的观察针刀治疗对退变兔腰椎间盘组织中凋亡调控基因Bcl-2和Bax表达的影响。方法将24只雄性健康成年的新西兰大白兔随机分为空白组、假模型组、模型组和针刀治疗组4组,每组6只。模型组与针刀治疗组配戴轴向压力装置建立兔腰椎间盘退变模型。造模成功后1 d治疗组进行针刀手术,每隔1周进行1次治疗,共4次。治疗结束后,24只兔子全部处死,HE染色观察组织形态,蛋白免疫印迹(Western Blot)法检测B淋巴细胞瘤-2基因(Bcl-2)和BCL2关联X蛋白(Bax)的蛋白表达。结果腰椎间盘组织退变程度为:空白组≈假手术组<针刀治疗组<模型组。各组Bcl-2蛋白表达差异无统计学意义(P>0.05);模型组Bax蛋白表达水平显著高于空白组和假模型组(P<0.01),针刀治疗组Bax蛋白表达水平低于模型组(P<0.05);模型组Bcl-2和Bax蛋白表达的比例与空白组和假模型组相比显著降低(P<0.01),针刀治疗组Bcl-2和Bax蛋白表达的比例高于模型组(P<0.05)。结论针刀干预兔腰椎间盘退变可影响椎体组织调节因子的表达,减少细胞凋亡,纠正基质降解与合成代谢之间的不平衡,延缓椎间盘退变,促使退变的椎体组织得到修复与再生。Objective To observe the effect of acupotomy on the expression of apoptosis regulating genes Bcl-2 and Bax in lumbar intervertebral disc tissue of rabbits with degenerative diseases.Methods Twenty-four healthy male New Zealand white rabbits were randomly divided into blank group,sham model group,model group and acupotomy treatment group,with 6rabbits in each group.Model group and acupotomy group were fitted with axial pressure device to establish the rabbit lumbar disc degeneration model.One day after the successful modeling,treatment group underwent acupotomy surgery,once every week for a total of 4 times.After treatment,all 24 rabbits were sacrificed,histological morphology was observed by HE staining,and protein expression of Bcl-2 gene(Bcl-2)and BCL2-associated X protein(Bax)was detected by Western Blot.Results The degenerative degree of lumbar intervertebral disc tissue was blank group≈sham operation group<acupotomy group0.05);the expression level of Bax protein in model group was significantly higher than that in blank group and sham group(P<0.01),and the expression level of Bax protein in acupotomy group was lower than that in model group(P<0.05);the protein expression ratio of Bcl-2 and Bax in model group was significantly lower than that in blank group and sham group(P<0.01),and the protein expression ratio of Bcl-2 and Bax in acupotomy group was higher than that in model group(P<0.05).Conclusion Acupotomy intervention on lumbar intervertebral disc degeneration in rabbits can affect the expression of vertebral body tissue regulatory factors,reduce apoptosis,correct the imbalance between matrix degradation and anabolism,delay intervertebral disc degeneration,and promote the repair and regeneration of degenerative vertebral body tissue.
