桂枝茯苓丸加味联合氯吡格雷治疗痰瘀阻络型脑卒中有效性观察

摘　　要：目的观察桂枝茯苓丸加味联合氯吡格雷治疗对脑卒中患者的有效性。方法将我院收治的98例缺血性脑卒中患者随机分为观察组与对照组各49例。对照组在常规治疗的基础上口服氯吡格雷,观察组在此基础上联合桂枝茯苓丸加味治疗,均连续治疗2个疗程后评估临床疗效。于治疗前后检测两组血浆黏度、红细胞比容(HCT)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平,采用美国国立卫生研究院卒中量表(NIHSS)评分、日常生活活动能力(ADL)量表评估两组神经功能缺损情况及日常生活活动能力,观察治疗期间药物不良反应情况。结果治疗后,观察组总有效率显著高于对照组(P<0.05);两组患者血浆黏度、HCT、TNF-α、IL-6指标及NIHSS评分均较治疗前明显下降,ADL评分较治疗前明显上升(P<0.05),且观察组上述指标较对照组同期更优(P<0.05);治疗期间两组均未出现严重不良反应。结论桂枝茯苓丸加味联合氯吡格雷治疗脑卒中临床疗效更优,能显著改善患者血液黏度、炎症反应程度及神经功能情况,且安全性较高。Objective To observe the effectiveness of Modified Guizhi Fuling Pills combined with clopidogrel in treatment of stroke.Methods A total of 98 patients with ischemic stroke were randomly divided into observation group and control group,49 cases in each group.Control group was given oral clopidogrel on the basis of routine treatment,while observation group was given Modified Guizhi Fuling Pills on the basis of control group.The clinical curative effect was evaluated after 2 courses of treatment,the levels of plasma viscosity,hematocrit(HCT),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)before and after treatment were detected,the neurological deficits and activities of daily living were evaluated by National Institute of Health Stroke Scale(NIHSS)and Activity of Daily Living Scale(ADL);the adverse drug reactions during treatment were observed.Results the total response rate in observation group was significantly higher than that in control group(P<0.05):plasma viscosity,HCT,TNF-α,IL-6 and NIHSS score were significantly decreased,while ADL score was significantly increased in both groups(P<0.05),the indexes above were better in observation group than control group(P<0.05);during treatment,there were no severe adverse reactions in either group.Conclusion The clinical curative effect of Modified Guizhi Fuling Pills combined with clopidogrel is better on stroke,which can significantly improve blood viscosity,inflammation response and nerve function of patients,with relatively higher safety.
