柔肝化纤颗粒联合特利加压素治疗肝硬化腹水临床疗效及对门静脉压力和氧化应激水平的影响

摘　　要：目的观察柔肝化纤颗粒联合特利加压素治疗肝硬化腹水临床疗效。方法将我院148例肝硬化腹水患者随机分为治疗组与对照组各74例。对照组接受特利加压素治疗,治疗组在对照组治疗基础上联合柔肝化纤颗粒治疗。治疗2周后评价疗效,比较两组治疗前后身体一般情况(腹围、腹水深度、24h尿量)、肝功能指标[总胆红素(TBIL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)]、门静脉压力指标(门静脉血流量、血流速度及主干内径)、氧化应激指标[丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)]、不良反应。结果治疗后,治疗组总有效率高于对照组(P<0.05);两组腹围、腹水深度、TBIL、ALT、AST、门静脉血流量、门静脉主干内径、MDA显著降低,24h尿量、门静脉血流速度、GSH-Px、SOD显著升高(P<0.05),且治疗组上述指标改善程度优于对照组(P<0.05);治疗期间,两组均未出现严重不良反应。结论柔肝化纤颗粒联合特利加压素能改善肝硬化腹水患者身体一般情况,修复肝功能,调节门静脉压力,缓解氧化应激,疗效确切,值得推广。Objective To observe thetherapeutic effect of Rougan Huaxian Granules combined with terlipressin on cirrhotic ascites.Methods A total of 148 patients with cirrhotic ascites were randomly divided into control group and treatment group.Control group was treated with terlipressin,while treatment group was additionally treated with Rougan Huaxian Granules.After2 weeks of treatment,curative effects,general physical conditions(abdominal circumference,ascites depth,24 h urine volume),liver function indexes(ALT,TBIL,AST),portal pressure indexes(portal vein velocity,portal vein blood flow,portal vein trunk diameter),oxidative stress indexes[malondialdehyde(MDA),glutathione peroxidase(GSH-Px),superoxide dismutase(SOD)]and adverse reactions were evaluated.Results After treatment,the total effective rate of treatment group was higher than that of control group(<0.05);abdominal circumference,ascites depth,TBIL,ALT and AST,portal vein blood flow,portal vein trunk diameter,MDAwere significantly decreased,while 24 h urine volume,portal vein velocity,GSH-Px and SOD were increased in both groups(<0.05),and improvement of the above indexes was better in treatment group than control group(<0.05).Conclusion Rougan Huaxian Granules combined with terlipressin can improve general physical conditions of patients with cirrhotic ascites,repair liver function,regulate portal pressure and relieve oxidative stress with significant curative effect.
