乌梅丸加减外用联合氯雷他定对血液透析后皮肤瘙痒患者肾功能、钙磷代谢的影响

摘　　要：目的观察乌梅丸加减外用联合氯雷他定对血液透析皮肤瘙痒(HDIS)患者肾功能、钙磷代谢的影响。方法将我院80例HDIS患者随机分为联合组与西药组各40例。两组均采用血液透析和基础治疗,西药组服用氯雷他定治疗,联合组采用乌梅丸加减外用联合氯雷他定治疗。比较两组临床疗效,两组匹兹堡睡眠质量指数(PSQI)、视觉模拟瘙痒(VAS)评分,血清血肌酐(Scr)、尿素氮(BUN)、血钙、血磷、全段甲状旁腺激素(iPTH)水平,皮肤病生活质量指数(DLQI)评分。结果治疗后,联合组临床总有效率显著高于西药组(P<0.05);两组PSQI、VAS评分较治疗前下降,两组血清Scr、BUN、血钙、血磷、i PTH水平较治疗前下降,两组DLQI每个维度评分、总分均较治疗前下降,联合组VAS、PSQI,血清Scr、BUN、血钙、血磷、i PTH水平,DLQI每个维度评分、总分均较西药组降低明显(P<0.05)。结论乌梅丸加减外用联合氯雷他定明显改善HDIS患者症状,改善睡眠质量,肾功能和钙磷代谢,提高生活质量,具有较好的疗效。Objective To observe the effects of Modified Wumei Pills combined with loratadine on renal function and calcium and phosphorus metabolism in patients with pruritus on hemodialysis(HDIS).Methods Eighty HDIS patients were randomly divided into joint group and western medicine group,40 cases in each.Both groups were treated with hemodialysis and basic treatment,western medicine group was treated with loratadine,and the joint group was treated with Modified Wumei Pills on the basis of western medicine group.The clinical efficacy of the two groups was compared,and the scores of Pittsburgh Sleep Quality Index(PSQI),Visual Analog pruritus(VAS),serum creatinine(Scr),blood urea nitrogen(BUN),serum calcium,serum phosphorus,Dermatosis Quality of Score of Life Index(DLQI),impact-segment parathyroid hormone(i PTH)levels were observed between the groups.Results After treatment,the clinical efficacy of joint group was significantly higher than that of western medicine group(P<0.05);the PSQI and VAS scores were decreased in both groups compared with those before treatment,and were significantly lower in joint group than western medicine group;the serum Scr,BUN,blood calcium,blood phosphorus and i PTH levels were decreased in both groups compared with those before treatment,and were significantly lower in joint group than western medicine group;the scores of each dimension and the total score of the DLQI in joint group were significantly lower than those in western medicine group(P<0.05).Conclusion Modified Wumei Pills combined with external application of loratadine can significantly improve the symptoms of HDIS patients,improve sleep quality,renal function and calcium and phosphorus metabolism,and improve the quality of life,which has a good curative effect.
