针刺后溪穴不同深度对治疗神经根型颈椎病的疗效探索

摘　　要：目的观察针刺后溪穴不同深度对治疗神经根型颈椎病的疗效影响。方法将我院神经根型颈椎病患者90例随机分为常规组、深刺组与浅刺组各30例,常规组给予针刺后溪穴20 mm治疗,深刺组给予针刺后溪穴30 mm治疗,浅刺组给予针刺后溪穴5 mm治疗,观察比较各组治疗后的临床疗效以及治疗前后颈痛量表(NPQ)评分、视觉模拟评分法(VAS)评分、肌电图指标(F波波幅、F波传导速度)变化情况。结果经治疗后,各组在治疗总有效率、NPQ评分、VAS评分、F波波幅、F波传导速度上差异均具有统计学意义(P<0.05);组内比较,各组NPQ评分、VAS评分均明显低于治疗前(P<0.05),F波波幅、F波传导速度明显高于治疗前(P<0.05);两组间比较,深刺组NPQ评分、VAS评分明显低于常规组、浅刺组(P<0.05),治疗总有效率、F波波幅、F波传导速度明显高于常规组、浅刺组(P<0.05),常规组与浅刺组在治疗总有效率、F波波幅、F波传导速度、NPQ评分、VAS评分均无明显差异(>0.05)。结论深刺后溪穴治疗神经根型颈椎病患者的疗效显著,可减轻患者疼痛,改善肌电生理,效果明显优于常规针刺与浅刺后溪穴。Objective To observe the curative effect of acupuncture at different depths of Houxi point on cervical spondylotic radiculopathy(CSR).Methods Ninety CSR patients were randomly divided into routine group(acupuncture at Houxi point for 20 mm depth),deep acupuncture group(acupuncture at Houxi point for 30 mm depth)and shallow acupuncture group(acupuncture at Houxi point for 5 mm depth),30 cases in each group.The clinical curative effect after treatment,changes in scores of Neck Pain Questionnaire(NPQ)and Visual Analogue Scale(VAS),and Electromyogram Indexes(amplitude and conduction velocity of F wave)before and after treatment were observed and among the three groups.Results After treatment,the differences among the three groups in total response rate of treatment,scores of NPQ and VAS,amplitude and conduction velocity of F wave were statistically significant(P<0.05);the scores of NPQ and VAS were significantly decreased(P<0.05),which were significantly lower in deep acupuncture group than routine and shallow acupuncture groups(P<0.05),while amplitude and conduction velocity of F wave were significantly increased in the three groups(P<0.05),and were significantly higher in deep acupuncture group than routine and shallow acupuncture groups(P<0.05);there were no significant differences between routine group and shallow acupuncture group in total response rate of treatment,amplitude and conduction velocity of F wave,scores of NPQ and VAS(P>0.05).Conclusion The curative effect of deep acupuncture at Houxi point is significant on CSR.It can alleviate pain and improve electromyography,whose effect is significantly better than those of routine acupuncture and shallow acupuncture.
