摘  要:目的探讨选择性水激发(WE)和频率衰减反转恢复(SPAIR)脉冲序列压脂技术在胸部前纵隔疾病扩散加权成像(DWI)中的应用价值。方法采用3.0T(Verio,Siemens)MR扫描仪对38例胸部前纵隔占位患者进行MR DWI扫描2次,分别采用SPAIR和WE压脂技术,比较b=0、150、900 s/mm^(2)的图像占位信号特点,并对脂肪抑制、伪影程度和病灶显示程度做主观评分,采用Wilcoxon符号秩和检验;测量DWI(SPAIR)和DWI(WE)b=0、150 s/mm^(2)和900 s/mm2的图像的信噪比(SNR)和对比噪声比(CNR)值,采用Mann-Whitney U检验分析;测量2种压脂技术DWI图像纵隔占位的表观扩散系数(ADC)值,采用独立样本t检验分析两者ADC值之间的差异。结果DWI(SPAIR)扫描时间7 min 30 s,DWI(WE)扫描时间4 min 10 s,后者比前者节省了43.83%,两者图像在b=0、150 s/mm2和900 s/mm2时占位信号特点一致,脂肪抑制、伪影程度和病灶显示的主观评分差异均无统计学意义;两者图像在b=0、150、900 s/mm^(2)时的SNR和CNR值差异无统计学意义(P>0.05)。两者图像ADC值平均值分别为(2.23±0.90)×10^(-3)mm^(2)/s和(2.26±0.93)×10^(-3)mm^(2)/s,差异无统计学意义(P=0.872)。结论胸部前纵隔DWI检查中,压脂WE和SPAIR技术的图像质量主观和客观评价相同,但WE技术明显节省扫描时间,更推荐应用于临床。Objective To explore whether the selective water excitation(WE)fat-saturated technique used in the diffusion weighted imaging(DWI)scan of the anterior mediastinum lesion can achieve the same image quality as the spectral attenuated inversion recovery(SPAIR)pulse sequence on the basis of saving scanning time.Methods A 3.0T(Verio;Siemens)MR scanner was used to perform MR scans on 38 patients with occupying lesions in anterior mediastinum twice.SPAIR and WE were used for DWI,respectively.Then,the signals characteristics of occupying lesion were compared between the DWI(SPAIR)and DWI(WE),and subjective scores for fat-saturated,artifact degree,and lesion clarity were recorded,while a Wilcoxon signed rank test was used to evaluate their differences.The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)values were measured and analyzed by Mann-Whitney U test.The difference in the apparent diffusion coefficient(ADC)value of all the lesions between DWI(SPAIR)and DWI(WE)was tested by the independent-samples t test.Results The scan time of DWI(SPAIR)was 7 minutes and 30 seconds,meanwhile the DWI(WE)was 4 minutes and 10 seconds.The latter saved 43.83%scan time compared with the former.There were no significant differences of subjective scores for fat-saturated,artifact degree and lesion clarity.The comparations of SNR and CNR values of the two kinds of images were no significant difference either(P>0.05).There was no statistical difference in the ADC value between the DWI(SPAIR)and DWI(WE)image(P=0.872).Conclusion In this study,WE and SPAIR showed the same subjective and objective evaluation of image quality in DWI examination of anterior mediastinum occupying lesion,but WE saved scanning time significantly,so it is recommended for clinical application.
