摘 要:目的探讨脑肽节苷脂对颅脑损伤患者脑氧代谢及脑血流的影响。方法选择2020年2月—2022年2月本院收治的116例TBI患者作为研究对象,按照随机数表法分为对照组与观察组两组,每组各58例。对照组进行常规治疗,观察组则进行常规治疗联合脑肽节苷脂注射液治疗。比较两组治疗前后的NIHSS评分、GCS评分、MMSE评分表、SF-36评分、脑改良氧利用率(Brain modified oxygen utilization coefficient,BMO2UC)、动静脉血乳酸差值(Arteriovenous difference of lacti acid,VALa)水平。结果治疗前两组的NIHSS评分、GCS评分、MMSE评分表、SF-36评分、BMO2UC及VALa水平比较,差异无统计学意义(P>0.05),治疗后两组的NIHSS评分均低于治疗前,且观察组低于对照组;治疗后两组的GCS评分、MMSE评分表、SF-36评分、BMO2UC及VALa水平均高于治疗前,且观察组显著高于对照组,差异有统计学意义(P均<0.05)。结论在常规治疗方法基础上,联合脑肽节苷脂注射液治疗TBI,疗效较好,可显著改善患者的脑氧代谢及脑血流状态,在本类患者中的应用价值较高。Objective To investigate the effects of cerebroside and ganglioside on cerebral oxygen metabolism and cerebral blood flow in patients with craniocerebral injury.Methods 116 patients with TBI,those were treated in our hospital from February 2020 to February 2022,were enrolled as study subjects.They were divided into control group(n=58)and observation group(n=58)according to the random number table method.The control group were treated with routine treatment,and the observation group were treated with cerebroside and ganglioside injection on the treatment of control group.The NIHSS score,GCS score,MMSE score,SF-36 score,levels of brain modified oxygen utilization coefficient(BMO2UC)and arteriovenous difference of lactic acid(VAla)of two groups before and after the treatment were compared.Results The NIHSS score,GCS score,MMSE score,SF-36 score,levels of BMO2UC and VAla of two groups before treatment were compared,the differences were not statistically significant(all P>0.05).The NIHSS score of two groups after the treatment were lower than those before the treatment,and the score of observation group was significantly lower than that of control group;the GCS score,MMSE score,SF-36 score,levels of BMO2UC and VAla of two groups were higher than those before the treatment,and the results of observation group were significantly higher than those of control group,the differences were statistically significant(all P<0.05).Conclusions On the basis of conventional treatment,combined with cerebroside and ganglioside injection in treating TBI could achieve ideal curative effect,and it could significantly improve cerebral oxygen metabolism and cerebral blood flow,so it is of high application value for the patients.
