针刺触发点联合臭氧治疗女性慢性盆腔痛的临床效果

摘　　要：目的:探讨细银针针刺触发点联合臭氧治疗女性慢性盆腔痛的临床疗效。方法:将132例慢性盆腔痛病人随机分为3组,各44例,A组给予细银针针刺触发点联合臭氧治疗,B组给予单纯阴部神经阻滞联合臭氧治疗,C组给予电刺激-生物反馈治疗。对比3组治疗前、治疗结束、治疗后1周、治疗后2周的VAS评分、临床总有效率、病人盆底Ⅰ类肌力及Ⅱ类肌力改善效果。采用SDS、SAS及SF-36于治疗前及治疗后2周对病人进行心理评分和生活质量评分。结果:治疗前3组病人各指标差异均无统计学意义(P>0.05),治疗后不同时间点,3组病人VAS评分均较治疗前下降(P<0.01),且A组病人VAS评分均低于其他2组(P<0.01);治疗后,A组病人临床治疗总有效率高于C组,盆底Ⅰ类和Ⅱ类肌力总有效率、SF-36评分高于B组和C组,SDS及SAS评分低于B组和C组(P<0.05~P<0.01)。结论:针刺触发点联合臭氧治疗女性慢性盆腔痛临床疗效较好,盆底肌力明显增强,有助于盆底功能恢复。Objective:To explore the clinical efficacy of trigger point acupuncture with fine silver needle combined with ozone in the treatment of female chronic pelvic pain.Methods:A total of 132 patients with chronic pelvic pain were randomly divided into three groups,with 44 patients in each group.Group A was treated with trigger point acupuncture with fine silver needle combined with ozone,group B was treated with simple pudendal nerve block combined with ozone,and group C was treated with electrical stimulation-biofeedback.The VAS score,clinical total effective rate,and the improvement effect of pelvic floor typeⅠmuscle strength and typeⅡmuscle strength were compared among the three groups before treatment,after treatment,1 week after treatment,and 2 weeks after treatment.In addition,SDS,SAS and SF-36 were used to evaluate the psychological score and quality of life score of patients before and 2 weeks after treatment.Results:There was no statistically significant difference in each index of patients in the three groups before treatment(P>0.05).At different time points after treatment,the VAS score of patients in the three groups decreased compared with that before treatment(P<0.01),and the VAS score of patients in group A was lower than that of the other two groups(P<0.01).After treatment,the clinical total effective rate in group A was higher than that in group C,the total effective rate of pelvic floor typeⅠand typeⅡmuscle strength,SF-36 scores were higher than those in group B and group C,and the SDS and SAS scores were lower than those in group B and group C(P<0.05 to P<0.01).Conclusions:Trigger point acupuncture combined with ozone has good clinical effects on female chronic pelvic pain,and the pelvic floor muscle strength is obviously enhanced,which is conducive to the recovery of pelvic floor function.
