摘  要:目的分析传染性单核细胞增多症(IM)患儿外周血中淋巴细胞亚群及NK细胞计数及比例的变化,探讨淋巴细胞亚群及NK细胞检测对IM患儿的临床意义。方法选择2020年3月—2021年7月本院儿科收治的IM患儿21例作为IM组,同时选择同期在本院儿科就诊的其他疾病患儿20例作为对照组。比较两组患者之间CD3^(+)T细胞、CD3^(+)CD4^(+)T细胞、CD3^(+)CD8^(+)T、CD19+B细胞及CD16^(+)CD56^(+)NK细胞计数和百分比的变化,并比较他们之间的差异。结果IM组患儿CD3^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞及CD16^(+)56^(+)NK细胞计数值均高于对照组,差异具有统计学意义(P<0.05),并且IM组CD4^(+)T细胞/CD8^(+)T细胞比值倒置;两组患儿之间CD19B+细胞计数变化不明显,差异不具有统计学意义(P>0.05)。IM组和对照组相比CD3^(+)T细胞、CD8^(+)T细胞百分率增加,CD4^(+)T细胞、CD19+B细胞、CD16^(+)CD56^(+)NK细胞百分率下降,上述差异均具有统计学意义(P<0.05)。结论检测外周血淋巴细胞亚群及NK细胞在辅助诊断传染性单核细胞增多症中具有较高的临床应用价值。Objective To analyze the variation of lymphocyte subsets and NK cell count and proportion in peripheral blood of children with infectious mononucleosis(IM),and to discuss the clinical significance of detecting the lymphocyte subsets and NK cell in peripheral blood in children with infectious mononucleosis.Methods 21 children suffered from IM,those were admitted to pediatric department of the hospital during March,2020 and July,2021,were enrolled in IM group.Another 20 children visited pediatric department of the hospital due to other disease during the same period were enrolled in control group.The variation of count and proportion of CD3^(+)T cells,CD3^(+)CD4^(+)T cells,CD3^(+)CD8^(+)T cells,CD19+B cells,and CD16^(+)CD56^(+)NK cells were observed,and the differences between the two groups were compared.Results The numbers of CD3^(+)T cells,CD4^(+)T cells,CD8^(+)T cells and CD16^(+)CD56^(+)NK cells in IM children were significantly higher than those in control group(P<0.05).The CD4^(+)T cell/CD8^(+)T cell ratio was inverted in the IM group.There was no significant difference in change of CD19+B cells between the two groups(P>0.05).The proportions of CD3^(+)T cells and CD8^(+)T cells were increased in IM group,while the proportions of CD4^(+)T cells,CD19+B cells and CD16^(+)56^(+)NK cells were decreased,the differences above were statistically significant(P<0.05).Conclusions Detecting the lymphocyte subsets and NK cells in peripheral blood of children plays an important role for diagnosing the disease of IM.
