优化主诊医师责任制管理方式对医疗资源消耗的影响

摘　　要：目的:探讨优化主诊医师责任制管理模式后对医院医疗资源消耗的影响。方法:采用自身前后对照研究,选取某三级综合性医院168名主诊医师进行研究,于2021年1〜12月对原有主诊医师责任制管理方式进行优化调整,2020年1〜12月采取既往主诊医师责任制管理模式,分别从优化前后患者的平均住院日、时间消耗指数、医疗费用消耗指数、耗材消耗指数、疾病复杂指数、药品消耗指数、医技消耗指数、次均费用几个方面进行对比。结果:采用新的主诊医师责任制管理方式后,患者平均住院床日、时间消耗指数、医疗费用消耗指数、药品消耗指数、医技消耗指数、次均费用均低于干预前,差异均具有统计学意义(P<0.05)。结论:优化主诊医师责任制管理方式有利于提高医生的责任心,强化科主任对科室的管理水平,在不降低收治疾病复杂程度的情况下,减少疾病对医疗资源的消耗。Objective:To explore the influence of attending physician responsibility system management on medical resource consumption.Methods:Using its own before-and-after control study,168 attending physicians in a Grade III hospital were selected for the study,and the original attending physician responsibility management mode was optimized and adjusted from January to December 2021,and the previous attending physician responsibility management mode was adopted from January to December 2020,and the average hospital stay,time consumption index,medical cost consumption index,consumables consumption index,disease complexity index,drug consumption index,medical technology consumption index,and sub-average cost of patients before and after optimization were compared,respectively.Results:After adopting the new management method of the attending physician responsibility system,the average hospital bed days,time consumption index,medical expense consumption index,drug consumption index,medical technology consumption index,and average cost per time were all lower than those before the intervention,and the differences were statistically significant(P<0.05).Conclusion:Optimizing the responsibility system management mode of the attending physician is conducive to improving the sense of responsibility of doctors and reducing the consumption of medical resources by diseases without reducing the complexity of the disease.
