摘 要:目的:探索分析数字医学技术辅助桡动脉掌浅支皮瓣设计在手部微型创面修复中的应用效果。方法:选择2020年1月至2022年8月在茂名市人民医院手外科应用游离桡动脉掌浅支皮瓣进行手部微型皮肤缺损创面修复的患者26例作为本次研究对象,按照手术方式的不同将其分为两组。其中一组为对照组,13例,采用常规的游离桡动脉掌浅支皮瓣进行修复治疗;另一组为观察组,13例,在数字医学技术的辅助下行游离桡动脉掌浅支皮瓣修复治疗。对比两组患者的手术时间、皮瓣危象发生率、皮瓣存活率,随访观察患者皮瓣的外观、感觉、功能及对供区的影响等。结果:观察组患者的皮瓣危象发生率低于对照组患者,皮瓣存活率高于对照组患者,差异有统计学意义(P<0.05)。两组患者的手术时间相比,差异无统计学意义(P>0.05)。通过随访得知,观察组13例患者的创面均达到甲级愈合,供区瘢痕轻度增生,功能良好;对照组中12例患者的创面达到甲级愈合,7例患者皮瓣稍臃肿,该组全部患者的供区瘢痕增生明显且伸腕功能稍欠充分,1例患者发生血管危象,需再次手术,1例患者发生皮瓣坏死,脱痂后瘢痕形成。结论:将数字医学技术辅助桡动脉掌浅支皮瓣设计应用于手部微型创面修复中,通过术前精确的评估与设计,指导皮瓣的切取,不仅能有效减轻供区损伤,还能显著提高皮瓣存活率,使手部微型创面修复效果更好、患手功能恢复更理想。Objective:To explore and analyze the application effect of digital medical technology assisted design of superficial radial artery flap in the repair of hand micro wound.Methods:A total of 26 patients who underwent the repair of micro skin defect wounds with free radial artery superficial branch flap in the hand surgery Department of Maoming People’s Hospital from January 2020 to August 2022 were selected as the subjects of this study.They were divided into two groups according to different surgical methods.One group was the control group,13 cases were repaired by the conventional free superficial branch radial artery flap.The other group was the observation group(13 cases),which was repaired with free radial artery superficial branch flap assisted by digital medicine technology.The operation time,incidence of flap crisis and survival rate of the two groups were compared.The appearance,feeling,function of the skin flap and its influence on the donor area were followed up.Results:The incidence of flap crisis in observation group was lower than that in control group,and the survival rate of flap was higher than that in control group,the difference was statistically signif icant(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).The follow-up showed that all 13 patients in the observation group had Grade A wound healing,mild hyperplasia of donor zone scar,and good function.In the control group,12 patients achieved Grade A wound healing,7 patients had slightly swollen flaps,all patients in this group had obvious scar hyperplasia in the donor area and slightly inadequate wrist extension function,1 patient had vascular crisis,requiring reoperation,1 patient had flap necrosis,and scar formation after scab removal.Conclusions:The application of digital medical technology assisted design of superficial radial artery flap in the repair of hand micro wound can not only effectively reduce the donor area injury,but also significantly improve the survival rate of the flap through acc
