摘　　要：目的:研究富血小板纤维蛋白(PRF)凝胶对糖尿病足溃疡(DFU)患者血管生成及炎症因子的影响。方法:选择2014年5月至2020年12月三河燕郊福合第一医院糖尿病诊疗中心收治的65例2型糖尿病(T2DM)合并DFU患者作为研究对象。根据治疗方法的不同,将其分为PRF组(n=33例)和对照组(n=32)。给予对照组常规治疗,PRF组在对照组的基础上加用PRF凝胶治疗。比较两组治疗第7 d、第14 d、第21 d的创面面积以及血管内皮生长因子(VEGF)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)的水平。结果:治疗第21 d时,PRF组的创面面积明显小于对照组,差异有统计学意义(P<0.05)。治疗第14 d和第21 d时,PRF组的VEGF水平比基线时(第0 d)均明显升高,TNF-α和IL-6水平比基线时均明显降低,且与对照组同时间点比较差异有统计学意义(P<0.05)。结论:PRF凝胶可促进组织血管生成,控制局部炎症反应,有利于DFU的愈合。Objective:To study the effects of platelet-rich fibrin(PRF)gel on angiogenesis and inflammatory factors in patients with diabetic foot ulcer(DFU).Methods:Sixty-five patients with type 2 diabetes mellitus(T2DM)complicated with DFU admitted to the Diabetes Treatment Center of Yanjiao Fuhe First Hospital of Sanhe from May 2014 to December 2020 were selected as subjects.According to different treatment methods,they were divided into PRF group(n=33 cases)and control group(n=32 cases).Control group was given routine treatment,PRF group was treated with PRF gel on the basis of control group.The wound area and the levels of vascular endothelial growth factor(VEGF),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were compared between the two groups on the 7th,14th and 21st days after treatment.Results:On the 21st day of treatment,the wound area of PRF group was significantly smaller than that of control group,the difference was statistically significant(P<0.05).On the 14th and 21st day of treatment,VEGF levels in PRF group were significantly increased compared with baseline(the 0th day),and the levels of TNF-αand IL-6 were significantly decreased compared with baseline,and the difference was statistically significant compared with control group at the same time point(P<0.05).Conclusion:PRF gel can promote tissue angiogenesis,control local inflammatory response,and facilitate the healing of DFU.
