固本祛风汤联合氯雷他定片治疗肺脾气虚型变应性鼻炎临床观察

摘　　要：目的观察固本祛风汤联合氯雷他定片治疗肺脾气虚型变应性鼻炎(AR)的临床疗效。方法将126例肺脾气虚型AR患者按照随机数字表法分为2组,对照组63例予氯雷他定片治疗,治疗组63例在对照组治疗基础上加固本祛风汤治疗,2组均4周为1个疗程,治疗3个疗程。比较2组疗效;比较2组治疗前后症状评分变化;比较2组治疗前后炎症因子水平、免疫球蛋白水平变化;比较2组不良反应发生情况。结果治疗组治疗后总有效率92.06%(58/63),对照组总有效率77.78%(49/63),治疗组疗效优于对照组(P<0.05)。2组治疗后鼻塞、喷嚏、流涕、鼻痒评分均较本组治疗前降低(P<0.05),治疗后治疗组鼻塞、喷嚏、流涕、鼻痒评分均低于对照组(P<0.05)。2组治疗后单核细胞趋化蛋白-1(MCP-1)、高迁移率族蛋白B1(HMGB1)、白细胞介素6(IL-6)水平均较本组治疗前降低(P<0.05),治疗后治疗组MCP-1、HMGB1、IL-6水平均低于对照组(P<0.05)。2组治疗后免疫球蛋白A(IgA)、IgG、IgM水平均较本组治疗前升高(P<0.05),治疗后治疗组IgA、IgG、IgM水平均高于对照组(P<0.05)。治疗组不良反应总发生率14.29%(9/63),对照组不良反应总发生率9.52%(6/63),2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论固本祛风汤联合氯雷他定片治疗肺脾气虚型AR,能改善患者症状,降低炎症因子水平,改善免疫功能,治疗安全性较好。Objective To observe the clinical efficacy of Guben Qufeng Decoction combined with loratadine tablets for allergic rhinitis(AR)of lung spleen-deficiency type.Methods Totally 126 AR patients(lung spleen-deficiency type)were randomly assigned to receive either loratadine tablets(the control group,n=63)or loratadine tablets+Guben Qufeng Decoction(the treatment group,n=63).The treatment continued for 3 courses(1 course for 4 weeks),the aim was to compare symptom scores,inflammatory factors,immunoglobulin.The curative effect and adverse reactions were assessed.Results The overall effective rate was higher in the treatment group(92.06%[58/63])compared with the control group(77.78%[49/63]),(P<0.05).After treatment,the scores of nasal congestion,sneezing,runny nose,nasal itching in groups were decreased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05).The monocyte chemoattractant protein-1(MCP-1),high mobility group protein B1(HMGB1),interleukin-6(IL-6)in groups were decreased after treatment(P<0.05),the reduction in the treatment group was more common compared with the control group(P<0.05).The immunoglobulin A(IgA),IgG,IgM in groups after treatment were increased(P<0.05),which were higher in the treatment group than in the control group(P<0.05).The difference was not statistically significant in the total incidence of adverse reactions between the treatment group and the control group(14.29%[9/63]vs 9.52%[6/63],[P>0.05],respectively).Conclusion For AR patients(lung spleen-deficiency type),Guben Qufeng Decoction combined with loratadine tablets can improve the symptoms of patients,reduce the level of inflammatory factors,improve immune function,and with good safety.
