凝血四项对妊娠晚期产妇产后出血的预防价值

摘　　要：目的探讨凝血四项[凝血酶原时间(PT)、国际标准化比值(INR)、纤维蛋白原(FIB)及D-二聚体(DD)]检测在妊娠晚期产妇产后出血中的预防价值。方法选取40例妊娠晚期产后出血产妇作为研究组;另选择同期本院进行分娩的40例妊娠晚期健康产妇作为参照组。比较两组凝血四项(PT、INR、D-D、FIB)检测结果。结果研究组PT、INR及D-D水平高于参照组,差异具有统计学意义(P<0.05);研究组FIB(202.82±38.42)mg/dl水平低于参照组(314.11±45.75)mg/dl,差异具有统计学意义(P<0.05)。Pearson相关性检验显示,妊娠晚期产妇产后出血的发生与PT、INR、D-D水平呈正相关,与FIB水平呈负相关(P<0.05)。结论PT、INR、FIB、D-D与妊娠晚期产妇产后出血发生具有相关性,其可作为预测产后出血的重要指标和评估妊娠期晚期产妇产后出血风险的重要依据,应用价值较高。Objective To investigate the preventive value of prothrombin time(PT),International normalized ratio(INR),fibrinogen(FIB)and D-dimer(D-D)in postpartum hemorrhage during the third trimester of pregnancy.Methods 40 patients with postpartum hemorrhage during the third trimester were selected as the study group.Another 40 healthy pregnant women who delivered in our hospital during the same period were selected as the reference group.The results of PT,INR,D-D and FIB were compared between the two groups.Results The levels of PT,INR and D-D in the research group were higher than those in the reference group,and the differences were statistically signifi cant(P<0.05).The FIB level of the study group(202.82±38.42)mg/dl was lower than that of the control group(314.11±45.75)mg/dl,and the difference was statistically signifi cant(P<0.05).Pearson correlation test showed that the occurrence of postpartum hemorrhage in the third trimester was positively correlated with the levels of PT,INR and D-D,and negatively correlated with the levels of FIB(P<0.05).Conclusion PT,INR,FIB,D-D are correlated with postpartum hemorrhage in late pregnancy,so they can be used as an important index to predict postpartum hemorrhage and an important basis to evaluate the risk of postpartum hemorrhage in late pregnancy.
