摘 要:在浅表组织热消融手术中,难以实时监测靶组织的温度场变化,从而无法准确地判断其热消融面积,且治疗参数的选择和热剂量预测主要依赖于临床医生的经验判断。该文针对浅表组织热消融手术中的治疗参数选择和热计量预测问题,利用Kwave仿真工具包构建聚焦超声换能器三维仿真模型,结合Westervelt方程和Pennes生物传热方程通过有限元法计算换能器辐照域内声场和温度场分布,比较介质物理属性的均匀和非均匀分布条件在3种辐照模式条件下声场和温度场的差异性,并在此基础上预测换能器辐照区域靶组织凝固性坏死面积,为术前临床医生选择合适的治疗参数和热剂量预测提供参考。In the superficial tissue thermal ablation surgery,it is difficult to monitor the temperature field of the target tissue in real time,so it is impossible to accurately judge the thermal ablation area,and the selection of treatment parameters and thermal dose prediction mainly depend on the experience of clinicians.In this paper,aiming at the selection of treatment parameters and heat metering prediction in superficial tissue thermal ablation surgery,a three-dimensional simulation model of focused ultrasound transducer is constructed using K-wave simulation toolkit,and the distribution of sound field and temperature field in the radiation domain of the transducer is calculated by finite element method combined with Westervelt equation and Pennes biological heat transfer equation.The differences of sound field and temperature field between uniform and non-uniform distribution of physical properties of medium under three irradiation modes were compared,and on this basis,the coagulative necrotic area of target tissue in the area irradiated by transducer was predicted.It provides a reference for clinicians to select appropriate treatment parameters and thermal dose prediction before operation.
