摘 要:目的探讨人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)诱发排卵不同状态对自然周期冻融胚胎移植(frozen-thawed embryo transfer,FET)临床结局的影响。方法选取2019年1月—2020年1月贵阳市妇幼保健院生殖中心收治的行自然周期冻融胚胎移植的患者108例,采取HCG诱发排卵,A组(54例)为自然排卵,B组(54例)为非自然排卵,比较两组患者的内膜厚度、移植胚胎数、优质胚胎数、排卵日和移植日的雌二醇(E2)、孕酮(P)水平以及妊娠结局。结果两组内膜厚度、移植胚胎数、优质胚胎数比较,差异无统计学意义(P>0.05);A组排卵日的E2(552.31±53.08)pmol/L、P(5.18±1.31)nmol/L,移植日的E2(557.26±52.12)pmol/L、P(57.19±14.11)nmol/L与B组排卵日的E2(554.29±54.36)pmol/L、P(5.20±1.30)nmol/L,移植日的E2(552.13±50.38)pmol/L、P(56.28±14.35)nmol/L比较,差异无统计学意义(t=0.192、0.080、0.520、0.332,P>0.05);两组临床妊娠率、流产率、异位妊娠率、活产率比较,差异无统计学意义(P>0.05)。结论采取HCG诱导排卵,自然周期冻融胚胎移植的妊娠结局不会受到排卵状态的影响。Objective To investigate the effect of ovulation induced by human chorionic gonadotropin(HCG)on the clinical outcome of frozen-thawed embryo transfer(FET).Methods A total of 108 patients who underwent natural cycle frozen-thawed embryo transfer in the Reproductive Center of Guiyang Maternal and Child Health Hospital from January 2019 to January 2020 were selected for ovulation induced by HCG.Natural ovulation occurred in group A(54 cases)and unnatural ovulation occurred in group B(54 cases).The intimal thickness,number of transferred embryos,number of superior embryos,estradiol(E2),progesterone(P)levels on ovulation and transplant days and pregnancy outcomes were compared between the two groups.Results Compared the intima thickness,number of transplanted embryos and number of high quality embryos between the two groups,the difference was not statistically significant(P>0.05).In group A,E2(552.31±53.08)pmol/L and P(5.18±1.31)nmol/L on ovulation day and E2(557.26±52.12)pmol/L and P(57.19±14.11)nmol/L on transplantation day,compared with E2(554.29±54.36)pmol/L and P(5.20±1.30)nmol/L on ovulation day and E2(552.13±50.38)pmol/L and P(56.28±14.35)nmol/L on transplantation day in group B,the difference was not statistically significant(t=0.192,0.080,0.520,0.332,P>0.05).There were no statistically significant differences in clinical pregnancy rate,abortion rate,ectopic pregnancy rate and live birth rate between two groups(P>0.05).Conclusion When ovulation was induced by HCG,the pregnancy outcome of natural cycle frozen-thawed embryo transfer was not affected by ovulation status.
