地佐辛联合帕洛诺司琼在使用卡前列素氨丁三醇剖宫产产妇中的应用

摘　　要：目的探讨地佐辛联合帕洛诺司琼在使用卡前列素氨丁三醇剖宫产产妇中的应用价值。方法选择2022年1—9月淄博市妇幼保健院收治的剖宫产术中给予卡前列素氨丁三醇后出现消化道不适者80例为研究对象。按照随机数表法分为两组,各40例。所有入组者均使用地佐辛,同时对照组联合使用甲氧氯普胺,观察组联合使用帕洛诺司琼。比较两组给予卡前列素氨丁三醇治疗期间不良反应、生命体征指标变化,统计两组疼痛及镇静评分,比较两组产后出血量及产后24 h子宫复旧高度变化。结果观察组中恶心呕吐、面色潮红、胸闷和呼吸增快的发生率2.5%低于对照组的35.0%,差异有统计学意义(χ^(2)=13.876,P<0.05)。干预后,观察组平均动脉压高于对照组,心率慢于对照组,差异有统计学意义(P<0.05)。观察组给予卡前列素氨丁三醇期间疼痛VAS评分为(3.5±0.6)分,低于对照组的(4.6±1.2)分,镇静评分为(4.1±0.2)分,高于对照组的(3.3±0.6)分,差异有统计学意义(t=5.185、8.000,P<0.05)。两组产后出血量与24 h子宫复旧高度比较,差异无统计学意义(P>0.05)。结论针对剖宫产产妇使用卡前列素氨丁三醇后应用地佐辛联合帕洛诺司琼,能有效预防使用卡前列素氨丁三醇后的不良反应,维持产妇生命体征平稳,降低疼痛程度,提高镇痛效果,且不影响子宫复旧。Objective To investigate the application value of dezocine combined with palonosetron in caesarean section of women who received carprost amine triol.Methods From January to September 2022,80 patients with digestive tract discomfort after administration of carboprost tromethamine during cesarean section admitted to Zibo Maternal and Child Health Hospital were selected as the study subjects.According to the method of random number table,they were divided into two groups,each with 40 cases.All participants were treated with dezocine,while the control group was treated with metoclopramide and the observation group was treated with paronorsetron.The adverse reactions and changes of vital signs during the treatment with carboprost tromethamine were compared between the two groups,the scores of pain and sedation were counted,and the amount of postpartum hemorrhage and the height of uterine involution at 24 hours postpartum were compared between the two groups.Results The incidence of nausea,vomiting,flushing,chest tightness and rapid breathing in the observation group was 2.5%lower than that in the control group(35.0%),the difference was statistically significant(χ^(2)=13.876,P<0.05).After intervention,the average arterial pressure in the observation group was higher than that in the control group,and the heart rate was slower than that in the control group,the difference was statistically significant(P<0.05).The VAS score of pain in the observation group was(3.5±0.6)points,lower than that in the control group(4.6±1.2)points,and the sedation score was(4.1±0.2)points,higher than that in the control group(3.3±0.6)points,the difference was statistically significant(t=5.185,8.000,P<0.05).There was no statistically significant difference in the amount of postpartum hemorrhage between the two groups and the height of uterine involution at 24 h(P>0.05).Conclusion Dezocine combined with palonosetron can effectively prevent the adverse reactions after the use of carprost amine triol,maintain the stable vital signs,reduce t
