颌面部皮肤毛母质瘤临床病理分析

摘　　要：目的:总结颌面部皮肤毛母质瘤的临床病理特点,提高诊断水平,减少误诊。方法:回顾分析54例颌面部皮肤毛母质瘤患者临床病理资料。结果:本组54例病例中,男性24例,女性30例,男女比例为1:1.24。发病年龄8个月~60岁,平均年龄19岁;20岁以前病例占73.1%。其中37.4%的病变位于腮腺区,33.3%位于面颊部,11.1%位于耳后区。仅有1例为多发。术前临床诊断正确率仅14.8%。所有病例均行手术切除,仅1例因手术不彻底而复发。确诊主要依靠组织病理学检查;组织学特征主要包括嗜碱性基底样细胞和影细胞。结论:毛母质瘤是少见的皮肤良性肿瘤,临床误诊率高,提高对其临床病理特征的认识有助于减少误诊。Objective:To summarize the the clinicopathologic features of pilomatricoma in the oral and maxillofacial region to strengthen the understanding and avoid misdiagnosis.Methods:The clinical manifestations and histopathological features of 54 cases of cutaneous pilomatrixoma in the oral and maxillofacial region were retrospectively analyzed.Results:Out of the 54 cases, 24 were male and 30 were female.The male to female ratio was 1:1.24.The age of diagnosis ranged from 8 months to 60 years of age(mean 19 years),and 73.1% was observed under 20 years of age.The most frequent locations were the parotid region(37.4%),followed by the cheek(33.3%)and postauricular region(11.1%).Only one case was multiple.A correct preoperative diagnosis was made in only 14.8% of cases.All lesions were excised, and only one patient had a recurrence after incomplete excision.The correct diagnosis depends on the histopathological examination.Histological features included basophilic basaloid cells and ghost cells.Conclusion:Pilomatrixoma is a rare benign cutaneous tumor with high clinical misdiagnosis rate.Recognization the clinicopathologic features of this tumor is helpful for oral and maxillofacial surgeon to avoid misdiagnosis.
