分析后路椎弓根螺钉内固定治疗腰椎骨折手术室护理要点

摘　　要：目的:研究腰椎骨折行后路椎弓根螺钉内固定治疗中手术室护理要点。方法:选取我院2017年11月-2020年12月行后路椎弓根螺钉内固定治疗的68例腰椎骨折患者为研究对象,按随机数字表法分为2组,各34例。对照组行常规护理,观察组行手术室综合护理,比对2组护理满意度、手术时间、住院时间、术中出血量、疼痛程度评分、腰椎功能评分及并发症情况。结果:护理总满意度对照组为73.53%,低于观察组的97.06%护理总满意率,比对结果(P<0.05);观察组手术时间为(98.33±7.69)分钟、住院时间为(10.29±3.24)天,均分别短于对照组的(115.26±7.26)分钟、(13.56±3.55)天,术中出血量为(32.48±2.65)ml,少于对照组的(52.36±5.42)ml,3项比对结果(P<0.001);观察组VAS评分术后7天为(2.06±0.17)分、14天为(1.01±0.12)分,均分别低于对照组的(3.12±0.24)分、(1.78±0.33)分,2项比对结果(P<0.001);对照组并发症发生率为29.41%,高于观察组的5.88%并发症发生率,比对结果(P<0.05);观察组JOA评分术后7天为(17.95±2.77)分、术后14天为(21.58±1.55)分及术后1个月为(26.59±0.66)分,均分别高于对照组的(15.98±2.85)分、(17.08±1.54)分及(20.12±0.57)分,3项比对结果(P<0.001)。结论:腰椎骨折行后路椎弓根螺钉内固定治疗中应用手术室综合护理,可有效缩短手术时间,改善患者腰椎功能,减轻术后疼痛感,以促进患者及早康复。Objective:To study the main points of operating room nursing in posterior pedicle screw internal fixation for lumbar fracture.Methods:A total of 68 patients with lumbar vertebral fracture who received posterior pedicle screw internal fixation in our hospital from November 2017 to December 2020 were selected as the research objects,and were divided into two groups according to random number table method,with 34 patients in each group.The control group received routine nursing,and the observation group received comprehensive nursing in the operating room.The nursing satisfaction,operation time,length of stay,intraoperative blood loss,pain degree score,lumbar function score and complications of the two groups were compared.Results:Compared with the control group(73.53%),the total satisfaction rate of nursing in the observation group(97.06%)was higher than that in the control group(P<0.05).The operation time(98.33±7.69)and hospital stay time(10.29±3.24)in the observation group were shorter than those in the control group(115.26±7.26).(13.56±3.55),the amount of intraoperative blood loss(32.48±2.65)was lower than that of the control group(52.36±5.42)(P<0.001).VAS score of observation group was lower than that of control group(3.12±0.24)at 7d and 14d after operation;(1.78±0.33),3 results(P<0.001);Compared with the control group(29.41%),the complication rate of observation group(5.88%)was lower(P<0.05).The JOA score of the observation group was higher than that of the control group(15.98±2.85)at 7d,14d and 1 month postoperatively.(17.08+/-1.54;20.12±0.57),3 comparison results(P<0.001).Conclusion:The application of operating room comprehensive nursing in posterior pedicle screw internal fixation treatment of lumbar fracture can effectively shorten the operation time,improve the lumbar function of patients,reduce postoperative pain,and promote the early recovery of patients.
