调理脾胃针法联合眼针疗法治疗DOP患者1例

摘　　要：目的 通过医案分析调理脾胃针法联合眼针疗法治疗糖尿病性动眼神经麻痹(DOP)的作用机理。方法 取调理脾胃针法之中脘、双侧足三里、三阴交,给予捻转补法,双侧血海、太冲,平补平泻,双侧曲池、合谷、阴陵泉、地机、丰隆,捻转泻法。患侧眼取眼针穴位之脾区、胃区、上焦区,平补平泻,治疗1次/d。结果 治疗10次后,患者动眼神经麻痹症状全部消失,治疗1个月,患者空腹血糖、餐后2 h血糖均降至正常,随访半年眼疾未复发。结论 调理脾胃针法能调理脾虚湿盛型糖尿病的脾胃升降功能,有效降低血糖水平,同时升高糖尿病眼病患者体内一氧化氮(NO)水平,降低内皮素(ET)水平,从而改善眼周循环;可以有效恢复周围神经的传导速度、波幅,缩短潜伏期,使动眼神经麻痹得以康复。眼针针法的脾区、胃区和上焦区,具有健脾益气、活血通络之功效,使眼脉得到脾胃气血的濡养。二针法合用,病机相合,共奏健脾养血、调理脾胃升降功能之效,远近结合,标本同治。Objective To analyze the action mechanism of diabetic oculomotor nerve paralysis(DOP)treating by regulating spleen and stomach acupuncture combined with eye acupuncture through medical records. Methods To take Zhongwan,Zusanli and Sanyinjiao on both sides from the acupuncture method of regulating spleen and stomach,and apply the twirling tonifying method;take bilateral Xuehai and Taichong, uniform reinforcing-reducing method was conducted;and take bilateral Quchi, Hegu,Yinlingquan,Diji and Fenglong,and apply the twirling reducing method. To take the spleen area,stomach area and upper energizer area of the eye acupuncture points of the affected side,and apply the uniform reinforcing-reducing method,and treat once a day. Results After 10 times of treatment,the symptoms of oculomotor nerve paralysis disappeared completely. After one month of treatment,the FBG and 2hPG of the patient decreased to normal,and the eye disease did not recur in half a year of follow-up. Conclusion Acupuncture for regulating the spleen and stomach could regulate the ascending and descending function of the spleen and stomach in diabetes with spleen deficiency and dampness excess,effectively reduce the blood sugar level,and at the same time increase the level of NO in the body of diabetes patients with ophthalmopathy,reduce the level of ET,thereby improving the circulation around the eyes. It could effectively recover the conduction velocity and amplitude of peripheral nerves,shorten the latency,and recover the oculomotor nerve paralysis. The spleen area,stomach area and upper energizer area of eye acupuncture had the effects of strengthening the spleen and replenishing qi,activating blood circulation and unblocking collaterals,so that the eye pulse could be nourished by the spleen and stomach qi and blood. The combination of the two acupuncture methods and the pathogenesis was the same. It could play a role in strengthening the spleen and nourishing blood,regulating the function of the spleen and stomach. It could cure the symptoms and roo
