体外冲击波配合针刺治疗脑卒中后上肢屈肌痉挛临床观察

摘　　要：目的观察体外冲击波配合针刺治疗脑卒中后上肢屈肌痉挛的临床疗效。方法首先,在脑卒中后上肢痉挛就诊患者中选取达到入选标准的60例患者,将其随机分为了对照组和治疗组,对照组首先进行常规的康复训练、针灸治疗,然后治疗组在对照组治疗的基础上增加对痉挛肌肉的体外冲击波疗法,2组各持续治疗4周,并分别在治疗前、治疗后2周、治疗后4周对患者进行简式Fugl-Meyer运动功能评价测量(上肢)(FMA)和改良Ashworth痉挛分级比较。结果2组患者持续治疗4周后,FMA、改良Ashworth痉挛等级均比治疗前有明显改善(P<0.01);组间对比显示,治疗后,治疗组患者的FMA和改良Ashworth痉挛等级改善明显优于对照组(P<0.01),差异均具有统计学意义。结论体外冲击波能有效提高脑卒中患者偏瘫上肢FMA评分,降低患侧上肢痉挛程度,改善患者偏瘫肢体功能。Objective To observe the clinical effect of extracorporeal shock wave therapy(ESWT)combined with acupuncture on flexor spasm of upper limb after stroke.Methods Sixty patients with upper limb spasm after stroke who met the inclusion criteria were selected.They were randomly divided into control group(group A)and treatment group(group B).Group A received conventional rehabilitation training and acupuncture treatment first,and then group B received extracorporeal shock wave therapy for spastic muscles on the basis of group A.Each group lasted for 4 weeks.Simplified Fugl-Meyer motor function assessment(FMA)and modified Ashworth spasm grading were compared before,2 weeks and 4 weeks after treatment,respectively.Results After 4 weeks of systematic treatment,FMA and modified Ashworth spasm grades in 2 groups were significantly improved compared with before treatment(P<0.01).Inter-group comparison showed that FMA and modified Ashworth spasm grades in the B group were significantly improved compared with the control group(P<0.01),and the differences were statistically significant.Conclusion ESWT can effectively improve the FMA score of hemiplegic upper limb in stroke patients,reduce the degree of upper limb spasm on the affected side,and improve the limb function of hemiplegic patients.
